The VDT Affinitē Risk Adjustment module includes: • Multi-user access to the stand-alone Risk Adjustment Module of the Affinitē platform for CMS HCC Risk Adjustment. With Affinitē Risk, HEALTH PLAN NAME will receive the following: o Implementation o Data Integration and Analytics, Warehousing and Daily Processing o Dashboard high-level insights including the status of HCC capture, the performance of providers and provider groups, and HCC opportunities o Reporting o Estimated RAF scores at the member, provider, and provider group level based on claims data o Estimated member level RAF score build-up incorporating demographics, acute and chronic HCCs, disease interactions, and HCC payment count o Identification of HCCs obtained from claims data and chart review o Visibility into detailed claim data, highlighting HCCs that impact the RAF score o Visibility into detailed pharmacy data including refill status and ordering provider o Comparison of estimated RAF scores for current and prior year(s) o Identification of Missing HCCs (year-over-year gaps) for all open years at the member level o Member level identification of possible suspect HCCs based on claims and supplemental data for all open years. o Preparation of RAPS claim submission files in CMS required format o Preparation of RAPS supplemental submission files from chart review in CMS required format o Ability to monitor providers and provider group related to HCC capture, HCC gaps and average RAF score o Train-the-trainer training and support on the VDT Affinitē solution

The VDT Affinitē Care Management includes the following capabilities will be included: • Dashboard with metrics from clinical, financial and operational areas, including care gaps, risk and population churn. (in development • Scorecard focused on plan and provider performance in NCQA HEDIS quality measures. Includes current performance, projected performance and performance against goal. Drill down to grouped metric for provider groups (TIN), individual provider (NPI) and individual plan member.(in development • Reports, offering both point in time and longitudinal reports in each area of focus – clinical performance, financial results, gaps in care, member activity and risk. In development • CareFlow rules engine providing automated identification, stratification / segmentation, notification, and workflow. Rules are easily customizable and offer an unlimited number of identifications, segmentation levels, and workflows. • Affinite CM is designed to be the “one-stop” shop for care managers. They are presented with pre-screened / selected target of members needing their help. They can see open cases and tasks. They can quickly access member information at several levels of detail, tailored for the task at hand. o Add boiler-plate notes o Add free-form notes o Assign members / cases to other stakeholders, indicating desired action and deadline o Communicate notes to member mobile o Communicate notes to member provider and other stakeholders o Maintain ongoing comprehensive assessment(s) o Maintain ongoing comprehensive care plan(s) o Complete, view, print notes and other documents (from a library of documents) o View full chart history based on aggregated data (professional, facility and medication claims) as well as member and other source’s reported information
