Patient Navigation

Docent Health's Patient Navigation Programs help health systems engage, activate, and build relationships with a diverse range of patient populations. We bring a mix of patient relationship management technology and hospitality-oriented services to provide patients with personalized support, at population scale. With our solution, health systems are able to meet and support patients where they are — whether that is by text, phone, email, or in the community. Our approach screens patients to close gaps in healthcare literacy, access, and social determinants that could derail the patient’s clinical journey. We help patients build the skills and confidence to productively engage with health and social support services. And, together with our health system partners, we build long term relationships with these patients that are based on trust, reciprocity, and great healthcare experiences.
