Medically Home a private firm based in Boston launched a digital technology-enabled system of care at home where physician and nurse practitioners use 2-way, biometrically-enhanced, two-way, audio-video systems to monitor and interact with patients from a command center. Medically Home also provides all bedside monitoring equipment and clinicians to make home visits, as well as delivery of supplies such as oxygen and diagnostics to the home. Medically Home care is designed as an alternative to both hospital and post hospital skilled nursing facility care. Under this model a much broader range of patients are eligible for acute home care. Medically Home pre-registers patients who have had high rates of inpatient care, allowing them to choose home care for future acute care needs. Medically Home launched its first program with Atrius Health System in Boston in the Spring of 2017 and has recently implemented a service offering post- him surgical care.
