“Our mission at NEVHC is to provide quality, safe and comprehensive healthcare to the medically underserved residents of our community. Leveraging our grant money by implementing Rimidi has enabled us to continue caring for our patients from the comfort and safety of their homes,” Alejandra Mata, MPH, CHES, Program Manager, Chronic Disease, NEVHC. “Looking ahead, we know that virtual methods of care, such as telemedicine and RPM, are here to stay and are effective when delivered appropriately. My hope is that Medicaid and other county health programs make telehealth reimbursement permanent, and have RPM devices covered by all insurances.”

"While Leon started remote patient monitoring with just glucometers for patients with T2D, it's important to note that many of these patients have multiple, comorbid conditions like obesity, heart failure, hypertension and fatty liver disease. A solution (like Rimidi) that supports multiple use-cases enables more personalized, holistic patient care. As you search for solutions, look for one that will support clinicians or clinical staff in managing comorbid chronic conditions side-by-side"

Finding partners that can effectively fit our healthcare delivery model has often been a challenge. However, Rimidi has provided us with a key patient service, allowing our organization to achieve yet another successful step towards the future of healthcare.”

"Technology may take time to implement procedurally with efficient workflow, but the right partner will help you devise strategies to use the technology efficiently and effectively.
We were able to demonstrate that using the technology tool, pharmacists were able to get patients to goal faster with A1c and lipid improvements above and beyond what was done with those pharmacists not using the too"

“Achievement of clinical goals in an efficient process and quicker time frame is good for our entire health team community. Rimidi is integrated with our NextGen EHR – not an easy task – and clinical data flows via cellular-enabled devices such as glucometers, scales and blood pressure cuffs back to our clinicians for rapid interpretation and intervention.”

As a safety-net hospital, many of our patients are underinsured and have social determinants that impact their healthcare, and monitoring postpartum BP through in-person visits can be challenging due to barriers in care access. Leveraging Rimidi and the connected blood pressure cuffs, which run on local cellular networks, our program has been able to significantly increase the number of BP measurements taken in the six weeks following delivery, a crucial component to proactively preventing adverse outcomes like heart attack and stroke.
