In development • CareFlow rules engine providing automated identification, stratification / segmentation, notification, and workflow. Rules are easily customizable and offer an unlimited number of identifications, segmentation levels, and workflows. • Affinite CM is designed to be the “one-stop” shop for care managers. They are presented with pre-screened / selected target of members needing their help. They can see open cases and tasks.
