DESCRIPTION

Use the device you want for any condition

Leveraging our health IoT platform and access to secure data from 600+ personal health devices, our remote monitoring application doesn’t lock patients and providers into a single device vendor or a few select brands. It offers maximum device flexibility to support remote care for chronic and post-acute conditions. From diabetes and hypertension to COPD and beyond, Validic® Impact remote monitoring manages multiple chronic care programs, concurrently or asynchronously, regardless of condition or population size.

Scale across conditions, populations

Organizations of all sizes can create and scale remote monitoring programs and offer meaningful, continuous and personalized care, across populations and chronic conditions. Standalone, disease-specific platforms are not necessary or economical. Organizations are not locked into a single vendor for every chronic condition or the need to budget multiple implementation and service integrations.

Improve operational efficiency with true integration

Many health IT vendors claim to “integrate” into the clinical workflow, but what they really do is “interface.” Our solution integrates directly into the clinical workflow, with standardized integrations through the Epic Connection Hub and Cerner Millennium®. Clinicians can enroll and monitor patients in an RPM program directly from their chart, launch our application from In-Basket messages, and streamline workflow by eliminating unnecessary clicks, toggles and uploads.

Generative AI eases the burden on clinicians

Our Generative AI summaries and period comparisons make getting up to speed and writing clinical notes a breeze. GenAI Automated RPM Summaries instantly executes a statistical analysis of the patient’s monitoring data and uses generative AI to create the basis of a clinical summary for the clinician, saving them precious minutes per encounter. GenAI Period Comparisons help you get up to speed quickly on what’s changed since last review, including what metrics are improving, declining, holding steady, and whether adherence is improving.

Because we know healthcare providers’ time is better spent with patients and not on administration, we designed our remote monitoring application to fit seamlessly into the systems they already know and use. Organizations don’t need to hire additional staff or allocate extra resources to RPM programs. Clinicians can view and manage critical data and notifications within the clinical workflow and view historical trends directly in the patient’s health record. They don’t need to be data scientists and will notice a reduction in redundant and inefficient manual tasks, ensuring they spend more time on what matters most - direct patient care.

Data Collection

Validic's remote care application supports more than 100 devices to allow patients to share biometric data from outside the hospital or clinic. It is extended to patients in two ways. First, it allows for a BYOD smartphone offering, where the patient can download our HealthBridge™ app and complete enrollment and device pairing on their personal device. Peripheral devices such as scales and blood pressure cuffs are provided by their care team or purchased by the patient online or in stores. The application also allows for the shipping of a pre-configured, locked-down tablet (kit), where all peripherals can be pre-paired and it "just works" out of the box. Whether provisioned from the BYOD or "kit-based" model, the data seamlessly flow from the patient's device and into the RPM program, where it is evaluated against programmed rules, is able to escalate alerts directly to providers or nursing queues, and is viewable and actionable within the EHR.

AVIA GUIDE
What is data collection?
The ability for individuals to use digital technology to share biometric and other health data in real-time from outside a hospital or clinic with their care team as a part of an ongoing provider-directed program of care
Analytics

The client configures Validic's program defaults, with customizeable rules at the program and individual patient levels. The data we receive from the patient's devices are evaluated against the program rules and the result of those evaluations can be alerts to providers, messages to specified individual or shared inboxes, follow-up surveys for more information, or messages sent directly to the patient with instructions on what actions they should take. Our clients build their programs with the appropriate measurement goals and thresholds to enable them to have awareness of any concerning trends in a patient's condition.

AVIA GUIDE
What is analytics?
The ability to identify patients who are at risk of a worsening condition/potential hospitalization
Bidirectional Communication

Validic's remote care application also includes asynchronous, conversational chat capability. They are used in two ways. First, to evaluate data received from a patient's device. For example, if a blood pressure reading is above the threshold set by the care team in that patient's program, the team can trigger a follow-up conversation to gather additional information from the patient such as worsening symptoms or medication adherence. Second, care teams can send chat conversations regularly (daily, weekly, etc) as a way to check in with the patient about their condition, progress in the program, or to gain other insights. The care team can build rules that evaluate the chat conversation responses and deliver alerts and escalations in a variety of ways, such as to a configured EHR message inbox. For example, an answer to a question of "are you feeling better/same/worse today" with an answer of "worse" could trigger a message for follow up.

AVIA GUIDE
What is bidirectional communication?
The ability to synchronously and/or asynchronously communicate between the patient and care team 24/7
Accessibility & Equity

Our patient-facing solutions can be offered on a pre-configured "zero setup" tablet, where the patient's peripheral devices are pre-paired and ready to go out of the box. The patient simply types in a PIN code and leaves the tablet plugged in/charged and in close proximity to the peripheral devices when taking readings. There are no other actions for the patient to take other than using their blood pressure cuff, scale, etc to take readings. For users who may already have their own glucometers, scales, or blood pressure cuffs, we support an expansive library of devices and continue to add more to our platform annually. This allows many patients to use the medical device with which they are already comfortable. Third, for patients who are comfortable using their own smartphone/tablet, we support Android and iOS through our HealthBridge™ native apps. After completing a simple, guided self-enrollment process, the patient is directed to pair their device, and readings seamlessly flow through the app as long as it's running in the background. The patient isn't required to open the app or take any additional actions beyond recording measurements using their peripheral devices in proximity of the phone where the HealthBridge™ app is installed. Finally, the HealthBridge™ app runs in English and Spanish.

AVIA GUIDE
What is accessibility & equity?
The ability to reach and engage any eligible patient, including those with limited technical skills, health literacy, and language skills
Pre-Built Care Plans

Starting with industry-standards-based Blueprints, our implementation team guides each customer through the creation and customization of care plans for each program into which that the customer is planning to enroll patients. These goals, rules, and alerts allow our customers to build highly specialized care plans that enable the care team to be alerted and informed regarding patients who need to be engaged, while giving them confidence on patients who are performing well in the program. Validic's remote care application allows for each program to have a default configuration that will apply to each enrolled patient, while also enabling the care team to modify thresholds and alerts on a patient-by-patient basis, to give as much flexibility as is needed to manage care.

AVIA GUIDE
What is pre-built care plans?
The ability to use ready-made, condition-specific, customizable care plans and pathways for clinical decision support and patient engagement
Integration & Interoperability

Validic's digital health and remote care platform is integrated with the Cerner and Epic EHRs, and NextGen Video Visit solution. Our integration includes: - Seamless integration of the provider clinical experience within the EHR via an embedded SMART on FHIR application. This includes SSO so there is never a need to have a second password. Providers work directly in the EHR and never have to "launch out" to a second application. - Enrollment of a patient into a program happens directly within the integrated experience. The care team can see which programs a patient is already enrolled into and enroll them into additional programs, all within the EHR embedded workflow. Data needed for enrollment is automatically read from the EHR, avoiding the need for the care team to manually type in demographic information. - The ability to generate inbox/inbasket messages directly into the EHR's native messaging system. This makes the information generated by remote care programs live alongside other messages that are being sent to the care team, including shared inboxes/inbaskets, to facilitate workflows where the care team is flexible throughout the day/week. - Writing the discrete result values received from the patients devices directly into the EHR flowsheet, so that it is evaluated by Validic's RPM rules within the program and also becomes part of the patient's medical record, to be used by other processes within the EHR as well as any downstream analytics, population health, HIEs, or other systems that depend on the EHR chart being comprehensive.

AVIA GUIDE
What is integration & interoperability?
The ability to seamlessly fit within IT infrastructure to improve the user experience
Operational Support

For higher-acuity programs or programs for individuals with lower technical abilities, Validic can provide everything from simple ship-and-forget clinical device fulfillment and IT setup and configuration to reverse logistics, friendly and compassionate 1-800 patient-facing technical support, and outbound onboarding calls to support a patient’s first experience with the programs. Staff training is part of our customer onboarding/go-live processes.

AVIA GUIDE
What is operational support?
The ability to support patients and care team with frictionless logistics management, staff training, and patient onboarding, and ongoing tech support, enhancing and not exacerbating the clinician experience

Differentiators vs internal options

Validic saw that many healthcare providers and payers struggled to manage data provided by aggregation platforms, so we built an entire operating system around our award-winning healthcare IoT platform. Impact prevents the need for numerous vendor relationships, buying cycles, implementations, or one-off workarounds to get patient-generated data operationalized in the clinical workflow. With Impact, organizations can move beyond one-off, disease-specific platform purchases. Instead, Validic Impact enables providers to scale implementations of disease management programs across disease states. Organizations are, therefore, not locked into a single vendor or the need to budget for multiple implementations and service integrations.

Differentiators vs competitors

Validic takes a platform-first approach to remote patient monitoring, setting it apart from condition-specific solutions and delivering programs at scale. We power the largest centralized RPM program in the nation, with more than 250,000 patients enrolled to date. Validic offers access to the widest range of devices and data sources, providing connections to more than 530 devices. Enabling remote monitoring capabilities within existing clinical systems and workflows, our remote monitoring solution enhances rather than detracts from substantial investments made in enterprise-wide solutions like the EHR. Administrators and physicians can configure the program the way they want, setting up personalized patient experiences and notifications to fit their preference. Our solution supports a bring-your-own-device model and a fully curated, ready-to-use kit mailed to the patient.

20 Most Popular Remote Monitoring Companies Among Rural Hospitals
Available digital healthcare solutions are evolving and improving rapidly, transforming how healthcare organizations meet patients’ needs. Few healthcare facilities have stood to benefit from the rise of telehealth technology as much as rural hospitals, which often serve as the primary source of outpatient services for underserved local communities. But recently, rural hospitals have struggled to sustain the high levels of outpatient care they typically provide. Additionally, hospitals in rural areas rank last when it comes to telehealth adoption, which has left them poorly prepared to take part in the shift to virtual-first care and put their financial health at risk. According to a recent McKinsey report, that will soon change, as rural hospitals increasingly turn to remote patient monitoring solutions to provide better, more accessible primary care and improve coordination with regionally scarce specialists. To do so, they’ll need to choose the right platform for their use case.

Q&A with Martha Lawrence of AccendoWave: Harnessing pain data to address health equity
AccendoWave’s Remote Monitoring Solution objectively measures pain and provides real-time pain data. Pain is one of the primary reasons patients access health care and a primary driver of cost, but since pain data can’t be extracted from electronic health records, health systems don’t have any data or insight about these costs. Additionally, pain has a bias problem: women and pain, ethnicity and pain, seniors and pain. Their number one complaint is often that their pain isn’t believed. If health care systems are serious about their health equity pledges, they must address pain bias with technology to validate patient pain. Retraining the entire clinical workforce just isn’t an option.

Q&A with Anish Sebastian of Babyscripts: Eliminating barriers to pregnancy care
Babyscripts offers a clinically validated obstetrics-specific solution for remote delivery of precision pregnancy care. Through a mobile app and remote monitoring experiences, it targets the problems generated from the critical shortage of obstetrical providers in the U.S., as well as systemic and structural failures that create barriers to care for low-income and minority women. Our solution addresses an outdated model for pregnancy care, social determinants of health barriers and an increasing demand for virtual care solutions. Across the country, hospitals are closing their OB or ED units, making it harder to access quality care for delivery and forcing parents to travel farther for their appointments. Half of rural counties do not have a practicing OB-GYN and more are lacking a midwife.

Frequently asked questions about remote patient monitoring
For decades, remote patient monitoring (RPM) consisted of paper logs to track vital signs and symptoms and phone calls between appointments. While blood pressure logs and weight tracking were useful for individual patients, there was no practical way to design, deploy and scale a remote patient monitoring program for an entire patient population. But rapid hardware and software innovation, coupled with the widespread adoption of desktop and mobile devices, made smart and efficient remote patient monitoring a reality.
