Achieving the IHI Triple Aim of reducing costs, improving health outcomes, and improving members’ experience of care requires coordination with all aspects of care, particularly for high-risk members. Our Care Coordination initiatives involve specially trained personnel dedicated to serving those members who have recurrent or complex transportation and health needs, especially dialysis patients, mental health patients, and children in prescribed pediatric extended care services.
