Description:

vitalera offers a wide range of use cases for remote patient monitoring (RPM), focusing on providing high-quality care through technology-driven solutions, with all the reimbursement processes automated together with AI analytics and alarms.

Our platform is disease-agnostic, meaning it can be applied across various medical conditions to improve patient outcomes. Here are the key use cases:

  1. Cardiovascular Care: vitalera helps cardiologists monitor patients with heart disease, hypertension, and arrhythmias remotely. By tracking vital signs such as blood pressure, heart rate, and oxygen levels, we enable early intervention, reduce hospital readmissions, and improve patient adherence to treatment plans, and AI alarms.
  2. Chronic Disease Management: For patients with chronic conditions like diabetes, COPD, and asthma, vitalera ensures continuous monitoring of vital signs, enabling healthcare providers to proactively manage symptoms, adjust medications, and minimize complications. This leads to better disease management and fewer emergency visits.
  3. Remote Post-Operative Monitoring: After surgery, vitalera provides tools to track recovery, monitor pain levels, and identify signs of complications early. This allows healthcare providers to respond promptly, minimizing the risk of readmission and improving recovery outcomes.
  4. Oncology Care: vitalera supports cancer patients by monitoring side effects, medication adherence, and overall well-being during chemotherapy or radiation therapy. This remote monitoring helps oncology providers adjust treatments in real time, ensuring a more personalized and effective care approach.
  5. Maternal Health: vitalera helps monitor high-risk pregnancies, including conditions like preeclampsia and gestational diabetes. By tracking vital signs, blood sugar levels, and other pregnancy-related factors, healthcare providers can ensure timely interventions and improve maternal and fetal health.
  6. Behavioral Health: vitalera supports patients with mental health conditions, providing continuous monitoring to track mood, stress levels, and overall mental well-being. This allows healthcare providers to offer more personalized care and ensure patients are staying on track with their treatment plans.
  7. Elderly Care: For aging populations or individuals with complex health needs, vitalera offers remote monitoring to track vital signs, medication adherence, and general health status. This enables elderly patients to live independently while ensuring they receive the care they need, reducing hospitalizations and enhancing quality of life.
  8. Sleep Disorders: vitalera supports patients with sleep apnea and other sleep disorders by monitoring their sleep patterns, oxygen levels, and other critical data. This helps healthcare providers adjust treatment plans like CPAP therapy, improving patient outcomes and sleep quality.
  9. Diabetes Management: We offer a solution for continuous glucose monitoring, helping patients with diabetes manage their blood sugar levels remotely. This data-driven approach provides real-time insights to help patients and healthcare providers optimize their treatment strategies.
  10. Pediatric Monitoring: vitalera offers remote monitoring solutions for pediatric patients with chronic conditions or post-surgical needs. This ensures that children are monitored effectively from the comfort of their homes, providing parents and healthcare providers with peace of mind.

In all cases, vitalera's easy-to-use platform integrates seamlessly with existing healthcare systems and is compliant with CPT codes for reimbursement, ensuring both providers and patients can benefit from cost-effective, high-quality remote monitoring.

Differentiators

Differentiators vs EHR Functionality:

Cadence is capable of two-way integration with Cerner, Epic, Athena, and other EMRs. Patient vitals and symptoms data captured by Cadence will be accessible via dashboards, notes, and reports in the Cadence platform and in our partners' physicians' EMRs.

Differentiators vs Competitors:

Cadence solves the three major challenges that have plagued RPM historically for our health system partners:

  1. RPM creates too much work for overburdened clinicians
  2. Narrow point solutions create the need for too many vendors
  3. Financial model lacks a clear return on investment

Cadence’s solution includes:

  1. Embedded Clinical Team. Cadence’s remote patient monitoring and responsive virtual care program is supported by an NP-led, embedded clinical team capable of administering care to their partners’ patients in accordance with disease state-specific protocols approved by each partner’s clinicians as part of the implementation process. These clinical protocols help PCPs intervene as necessary so that together, the Cadence Care team and the patient’s PCP, can make adjustments to the care plan for their chronic illness as needed. This results in more proactive, preventive patient care and a reduction in clinicians’ workload. Historically, the Cadence Care team has been able to resolve 99% of patient interactions without the need for escalation to the partner clinician.
  2. Flexible, End-to-End Technology. Cadence is built on top of a flexible technology infrastructure that adapts to support our partners’ remote monitoring and virtual care needs, including the ability to integrate directly into a partner’s EMR and incorporate new devices, use cases, levels of acuity, and care pathways. Cadence’s seamless technology and patient-centric user experience is driving industry-leading engagement today, with over 88% of patients sharing vitals at least 16 days per month.
  3. Low-Risk Financial Model. Cadence offers a low-risk financial model with fees tied to performance and actual patient utilization. Cadence makes the upfront capital investment to support the implementation of the remote monitoring program, including approved devices, deployment configuration, enrollment staff, clinical staff, technical support staff, and logistics. In both fee-for-service and value-based arrangements, Cadence only invoices partners for services rendered to the patients.
