Digital offers the ability to deliver behavioral health care, support a patients’ spiritual coping, and maximize their benefits (e.g., financial navigation, social needs, spirituality and mindfulness, behavioral health resources)
Caregiver Support

Koda's ACP allows patients to digitally send their plans to their Surrogate Decision Makers (SDMs) and any other caregivers. Our KodaCares service facilitates phone calls between SDMs, loved ones, and the patient to ensure clarity and alignment regarding care preferences.

Koda seamlessly fits within a system's workflow and needs. Once patients are referred to us, Koda's ACP experience guides them through their planning. Once their plans are complete, they're made digitally available to your organization. You'll receive regular updates about the progress of your patient base through ACP; we also notify you of patients appropriate for a DNR-OOH, and any needs that may become apparent while your high-risk patients are supported through our KodaCares service.

Digital capabilities to optimize care team workflows and improve care team experience (e.g., Care coordination, remote monitoring, virtual tumor boards, clinical decision support, clinical and quality analytics, care team communication)
Interactive & Multimodal

Koda's digital tools contain videos, "select all that apply" sections, and other interactive tools to educate patients then capture their care decisions. There are in-app nudges that have various functions including: 1) notifying patients when a decision differs from their ideal quality of life 2) Explain their options and the requirements for in-app legalization 3) Clarify surrogate decision-maker options and requirements. This interactive process aggregates these decisions into the appropriate legal forms without feeling as though they're completing paperwork.

Delivered via a variety of formats (e.g. video, audio, print, etc.) and modalities (e.g. website, mobile apps, etc.) to capture patient attention and improve comprehension. Drag-and-drop type functionality allowing patients to visualize their own medical conditions.
