Koda's ACP allows patients to digitally send their plans to their Surrogate Decision Makers (SDMs) and any other caregivers. Our KodaCares service facilitates phone calls between SDMs, loved ones, and the patient to ensure clarity and alignment regarding care preferences.

Koda seamlessly fits within a system's workflow and needs. Once patients are referred to us, Koda's ACP experience guides them through their planning. Once their plans are complete, they're made digitally available to your organization. You'll receive regular updates about the progress of your patient base through ACP; we also notify you of patients appropriate for a DNR-OOH, and any needs that may become apparent while your high-risk patients are supported through our KodaCares service.

Koda's digital tools contain videos, "select all that apply" sections, and other interactive tools to educate patients then capture their care decisions. There are in-app nudges that have various functions including: 1) notifying patients when a decision differs from their ideal quality of life 2) Explain their options and the requirements for in-app legalization 3) Clarify surrogate decision-maker options and requirements. This interactive process aggregates these decisions into the appropriate legal forms without feeling as though they're completing paperwork.

Koda Health simplifies the complicated language behind medical procedures and legal terminology to allow patients to self-navigate their care planning. Any in-app text is written at 6th-grade reading level, and our video explainers include captions and visuals to explain and capture any of the medical procedures and terminologies.

KodaACP is available on any internet-accessible device. KodaCares provides telephonic support for patients without these devices.

Koda's tools are designed and optimized for easy, self-navigation by a 65+ patient population. That means no downloads are required, easy accessibility using any internet-connected device, and a user interface meeting rigorous international requirements. For patients with additional needs whether that be literacy barriers, tech avoidance, or any other barriers, KodaCares guides them through this process telephonically.

Within our KodaCares service, our licensed clinical staff will guide patients through a social determinants of health (SDOH) intake and work with them and their care team to alleviate said barriers that may impact their care decisions.

Koda is HIPAA-compliant and SOC2-Type II compliant, ensuring that patient data remains secure and safe.

Patients can access and update their plan at any time by logging into Koda. Partner Health systems are given access to the most up-to-date plans as soon as they're updated by patients.
