Patient Completes Pre-Visit Assessment, the UI color codes patients Diabetes and Hypertension Risk in a Red/Yellow/Green interface , and recommends a bespoke remote monitoring program in the mobile/tablet UI. Patients are able to download the mobile app for their recommended program pre-visit, opting in to RPM patient consent in automated fashion.
