Description
Chart Access provides secure access to patient records as well as provider messaging for organizations providing care outside of a health system. This solution is currently available for health systems utilizing Cerner Millennium clinicals. When a patient is discharged to a nursing home, skilled nursing unit, etc. those providers at the post discharge facility need an easy way to access hospital and/or clinic records for the patients at their post discharge facility. They also need a way to efficiently communicate with health system providers that are visiting patients at their facility. Today that requires a lot of paper and
phone tag. Chart Access eliminates the need for paper exchange and playing phone tag.

Health systems have a need to share patient records and communicate with trusted providers and caretakers in order to better facilitate coordination of care.
Whether it be discharge care facilities, specialist providers, therapists, or a variety of providers, Chart Access provides secure access to patient records and messaging for organizations providing care outside of a health system. It also allows these providers to contribute limited documentation into the electronic medical record (EMR).
Pediatric use cases:
Because this product is not specific to securely viewing patient records for adults, this product will work for children as well who are treated in ancillary facilities to the EHR health system.
Users:
The end users of this product are clinicians outside the EHR health system where patients are discharged to or seek treatment in ancillary facilities.

Differentiators vs EHR Functionality:
Cerner's MPages Reach solution offers "read-only" access to a patient's record while Chart Access offers both "read-only" along with the ability to message back into the EHR system's native messaging platform. Additionally, Chart Access allows for clinical documentation to flow back into the EHR patient record.
Differentiators vs Competitors:
We're not aware of any current external competitors who offer a secure provider portal for post-acute care that integrates back into Cerner Millennium outside of Cerner's MPages Reach product.
