We have been using this software for our health system for near 8 years now. Has certainly helped drive down our readmissions rates and improved our value based care.
Pros:
-Simple patient-facing user interface, with the tablet providing step-by-step instructions
-Clinician user interface helps highlight issues and trends
-Price for this platform is quite competitive
Cons:
-Glucometry is a bit clunky
-Limited number of compatible bluetooth devices

The LifeStream website and the patient tablets are very easy to use. I personally have had many successes while using this product with my high risk heart failure patients.
Being able to video chat with my patients on these tablets has given me the opportunity to make a significant impact on my patients lives while improving their health outcomes.
I cannot forget to add a comment about the amazing clinical and customer support. Always answering the phone on the first ring, no matter the time of day or night, promptly responding at my time of need. Truly top notch.

The program has been an amazing addition for our patients who are high risk and vulnerable. It gives them the opportunity to feel more connected to their provider as well as an avenue to feel more heard about their health. It has kept patients out of the hospital and when they did have to go to the hospital, we were able to intervene before it was dire.
The system is easy to use and simple to tailor to your practice. This program has also come with a feature we didn't expect, their tech support. They are so incredibly helpful and easy to work with. There are patients who were going to disenroll until they had the pleasure of being in contact with the tech support. Their experience actually kept them in the program.
Our program doesn't have ability for EHR integration so I will not be adding it to the review. That's not Clear Arch's fault, we just don't have that feature we can use. Everything else has been amazing.
It has been a pleasure to interact with Clear Arch as a whole.
I would recommend this program to everyone who has the ability to impliment this program.

We use Clear Arch/LifeStream at our hospital for remote patient vital sign monitoring of about 150 patients per day. The platform is easy to use for patients and providers. Reporting some data from the site can be a little challenging but customer support is very willing to help with this. The client services team is also very helpful to work with.
