The PoC App provides clinical information - guidelines, pathways, triaging elements - to providers in highly contextualized and usable formats. This is the next-gen replacement for static, inaccessible, and unwieldy guidelines and flowcharts. The app iss currently being used standalone and via EHR integrations.

We are a team physicians and informaticians from Harvard, Yale, Columbia, and Mt. Sinai that transform clinical care pathways into interactive apps that clinicians love so provider organizations can ensure the practice of evidence-based medicine and enhance patient care.
