• Dashboard with metrics from clinical, financial and operational areas, including care gaps, risk and population churn. (in development
• Scorecard focused on plan and provider performance in NCQA HEDIS quality measures. Includes current performance, projected performance and performance against goal. Drill down to grouped metric for provider groups (TIN), individual provider (NPI) and individual plan member.(in development
• Reports, offering both point in time and longitudinal reports in each area of focus – clinical performance, financial results, gaps in care, member activity and risk. In development
• CareFlow rules engine providing automated identification, stratification / segmentation, notification, and workflow. Rules are easily customizable and offer an unlimited number of identifications, segmentation levels, and workflows.
• Affinite CM is designed to be the “one-stop” shop for care managers. They are presented with pre-screened / selected target of members needing their help. They can see open cases and tasks. They can quickly access member information at several levels of detail, tailored for the task at hand.
