A health system enrolls patients with uncontrolled hypertension into a remote patient monitoring (RPM) program using BPM Pro 2.
Upon enrollment, each patient receives a BPM Pro 2 device shipped directly to their home. Thanks to built-in cellular connectivity, patients can begin taking blood pressure measurements immediately—no setup, app download, or Wi-Fi required.
The device guides patients through proper measurement techniques (e.g., posture, rest time), helping ensure accurate readings. Each measurement is automatically transmitted to the care team in real time, along with contextual insights such as measurement conditions and trends over time.
Care teams monitor incoming data through their RPM platform and can quickly identify patients with elevated or worsening readings. For example, if a patient’s blood pressure remains consistently above target thresholds, the care team can intervene early—adjusting medications, providing lifestyle coaching, or scheduling follow-ups.
Over time, this continuous monitoring and feedback loop improves patient adherence, enables more timely clinical decisions, and helps bring blood pressure under control—reducing the risk of complications such as stroke or heart disease.
