The Welldoc Platform includes: 

  • An artificial intelligence (AI) driven digital coaching app, providing personalized, actionable guidance and longitudinal feedback based on the six dimensions of health (1) Symptoms, (2) Food, (3) Activity, (4) Pscho-Social, (5) Medications and (6) Labs to help support the patient throughout their chronic care journey. 
  • Integrated mental wellbeing support that educates individuals through their behavioral patterns to design a personalized strategy and action plan. 
  • Comprehensive Digital Coaching and Education based on clinical standards adapted and digitized from Centers for Disease Control and Prevention (CDC) and American Heart Association (AHA) programs 
  • Reporting and workflow integration enabling care teams to easily access data insights to inform clinical interventions and decisions. This includes our Management Portal that provides care management with enhanced population-level health data and analysis.

Welldoc offers reporting packages at the individual patient as well as population level. Patient Level Reporting: Health (Smart Visit) Reports – On a routine/scheduled basis, or as needed between visits, the Smart Visit Report will be shared with our client's clinical team to summarize the patient's current clinical situation, including clinical decision support grounded in evidence based guidelines for our client's clinicians to review suggested self-care behaviors, potential medication alternatives, etc. Patients can also view this report on demand through the patient facing application The Smart Visit Report includes: - 90-day cardiometabolic biometric data summary - Medication plan summary - Trends graphs giving immediate visibility to effectiveness of diabetes management plan and any particular time of day challenges - Clinical and patient self-management summary - Evidence-based clinical decision support recommending potential care plan adjustments based on the data for consideration by the HCP, citing clinical guidelines, and - Optional daily logbook, containing patient data for the past 30 days, including imported and manual cardiometabolic entries, medication adherence, sleep, activity, and member notes adding context to the data inputs. In addition to the Smart Visit Reports, patients receive weekly reports that provide insight into medication adherence, glucose trends, diet and activity summaries and allow them to plan for the upcoming week. The weekly report allows the patient to view weekly glucose patterns to inform and optimize treatment plans Population Level Reporting: Population reports are available to our customer stakeholders on a regular basis (typically monthly) and include the following information: 1. A good cardiometabolic condition management program uses effective communication and data collection technologies to provide ease of access to patients and to facilitate better risk stratification via the real-time data collection from physiological measurement devices or patient self-report metrics. From a cost-effectiveness perspective, it is important to provide a range of services to match patient need. Welldoc can either power our customer's Care Team or we can augment our technology platform with our own partner. When we bring our partner to an engagement, through the use of Welldoc’s technology and programs and pairing this with our clinical services team and experience, we have an excellent methodology for targeting higher touch, wrap around services for higher risk patients and lower touch, preventive services for lower severity, but still at-risk groups. Ultimately, we seek to ensure deployment of clinical resources to those patients that are at highest risk of increased emergency department and hospital utilization while working to lower risk in the entire population via preventive coaching and FDA cleared digital programs.

Bidirectional Communication

Welldoc offers the capability for in-app two-way bidirectional asynchronous chat for 1:1 communication between member and case/care managers that can be initiated by either member or case/care manager. We offer the option of secure asynchronous text or via video chat with one and/or both available to be “turned on” for our customers. In addition, our capability allows for population level messages to be sent to all or a cohort of members by their case/care manager through and in app messaging feature. Case/care managers can monitor incoming messages, respond to chats, initiate video calls, and generate in-app messages from the Welldoc care management portal.

Welldoc is committed to providing a product that can be accessible to members of all populations. We have demonstrated success in many hard to support populations including Medicaid, Medicare and rural (our most recent clinical study). Welldoc's FDA clearance and clinical rigor validates our dedication and commitment to overall public health. Language- All digital coaching through the Welldoc App is at a 5th grade reading level. The Welldoc solution is available in English and neutral Spanish. Welldoc also has the capability to expand our platform to other languages in the future. Device Agnostic The Welldoc application has cross-device operability with over 400 fitness trackers, blood glucose, continuous glucose monitoring, blood pressure, weight scales, labs and pharmacies. Full list available on request. Our platform being device agnostic allows plans to either keep their existing product formularies or when ready to change product formularies have the assurance that any brand selected will integrate with the Welldoc platform. Welldoc is not a strip company so we will not be changing members to a specific brand and we will not be shipping supplies on a monthly basis. Our goal is to reduce DME and Pharmacy spend. Members can keep their current devices and our client will not get any member complaints or service disruptions. SDOH Screening Welldoc strives to combat health iniquities to improve care through utilizing surveys to identify risk for depression, behavioral health needs, and social determinants of health. When members respond, the information is shared with identified client care teams/clinicians for outreach and support. Customer Crae Customer care supports members in both English and Spanish, with a language line available for others languages. Customer care supports account creation, onboarding, understanding app features, and syncing devices. Welldoc has a flexible integration approach Welldoc offerings include the ability to integrate directly into the client's case/care management solutions via Single Sign On (SSO) Integration allowing seamless experience to the care team end users. We support different SSO including SAML, OKTA etc. In addition, Welldoc also offers support via REST API and can easily integrate external APIs such as 'Eligibility API' for real time eligibility checking during patient onboarding and tailor the experience to each patient/user. For clients that do not have APIs we can also accommodate them using secure files exchange via SFTP. With the help of our strategic vendors such as Xealth, Redox or ZeOmega/Jiva (or other Care Management Platform vednors/technologies), we have clients where Welldoc is integrated directly into Care Management/EHR systems allowing write back capabilities via Flowsheet or In Basket messaging. As applicable for those who already have advanced member facing digital assets, we have also developed Software Development Kit (SDK) and API (Application Programming Interface) capabilities that enable the client to build in the Welldoc platform features and functionality directly into your digital assets. Our current set of SDKs are “UI-less” which allows you the flexibility to integrate based on your business needs. One of the major advantages of Welldoc platform is that it is device agnostic. The Welldoc platform leverages patients’ existing Smartphones and integrates with over 400+ connected devices and data sources, including: -Blood glucose meters -Blood pressure monitors -Weight scales -Fitness trackers -Continuous Glucose Monitors (CGM) A device agnostic approach allows individuals to use their existing technology which helps to overcome the inertia and challenges of learning how to use a new set of device(s) as part of starting a new program. We leverage an app based/smartphone driven solution to take advantage of the technology that most patients have with them throughout the day. Our solution also provides connectivity to pharmacies (CVS, Walgreens, Rite-Aid), laboratories (Quest, LabCorp) and patient portal data sources. A device agnostic approach fosters a relationship between your patients and their providers who can continue to prescribe formulary-covered devices that they and their patients are comfortable with. Use case descriptions

New York City Health and Hospitals (NYC H+H) with 273 total participants on BlueStar demonstrated reduction in A1c with an average six-month reduction in A1c (1.1 vs. 0.2; -0.9, p<0.01). There were high levels of engagement with more than half of the participants used the app once a week; recording blood glucose and medications were the most frequently used features (82% and 62% respectively). Of BlueStar participants, 40% were Spanish-speaking and received the Spanish-version of BlueStar. All English- and Spanish-speaking participants found BlueStar easy to use and understand.

Montana Rural Case Study

The state of Montana provided Welldoc’s digital health solution for diabetes, FDA-cleared BlueStar®, to eligible individuals with type 1 or type 2 diabetes* as part of its effort to scale DSMES services. The program sought to identify whether a digital tool could expand access to and participation in these services. It was an exploration of innovativeways to eliminate barriers for individuals with diabetes from participating in programs and improve retention as well as health outcomes. 

Results:

Glycemic Outcomes - An Average improvement of 47 mg/dL amongst engaged users

Blood Pressure Outcomes - An average reduction of 10 mmHg amongst those who showed reduction

Weight Outcomes - An average of 7% reduction of weigth from baseline amongst those who lost weight

Conclusion

An integrated digital health solution can support health systems and health plans in expanding access and extending existing care programs. The partnership between the state of Montana and Welldoc demonstrates the value of AIpowered digital coaching to meet program goals, guide individuals in their daily health, support positive outcomes,

empower care teams with data-driven insights, and scale care to broader populations.

Users

A user must be over age 18 and can self-identify their cardiometabolic chronic condition(s) for treatment such as diabetes, hypertension, heart failure, etc. If additional eligibility criteria are desired, Welldoc can work with a client to understand their desired eligibility guidelines.

Digital Coaching

Welldoc offers 1:1 asynchronous chat capabilities, two-way messaging, and live video meeting capabilities directly through our application. We enable this functionality for use by our clients’ Care Management teams, allowing them to function most efficiently in data-driven interventions that prioritize the members most in need.

Welldoc is a device-agnostic solution. The Welldoc platform connects to over 400 devices and data sources, including CGM, blood glucose meters, scales, blood pressure devices, fitness trackers, pharmacy and clinical laboratory data. Given Welldoc's device-agnostic approach, preferred supplies/consumable products are dictated and provided by the client. Welldoc can configure the app to support device/supply ordering functionality from within the application if so desired. This also prevents any interference with existing formulary strategies.

Integration into care models

Our Welldoc platform can push notifications, alerts, and reports to care team members and providers within their existing workflows with integration into their EMR or care management platform. Welldoc is capable of sending and receiving data to health systems. Integration details are handled at implementation.
