A good cardiometabolic condition management program uses effective communication and data collection technologies to provide ease of access to patients and to facilitate better risk stratification via the real-time data collection from physiological measurement devices or patient self-report metrics. From a cost-effectiveness perspective, it is important to provide a range of services to match patient need. Welldoc can either power our customer's Care Team or we can augment our technology platform with our own partner. When we bring our partner to an engagement, through the use of Welldoc’s technology and programs and pairing this with our clinical services team and experience, we have an excellent methodology for targeting higher touch, wrap around services for higher risk patients and lower touch, preventive services for lower severity, but still at-risk groups. Ultimately, we seek to ensure deployment of clinical resources to those patients that are at highest risk of increased emergency department and hospital utilization while working to lower risk in the entire population via preventive coaching and FDA cleared digital programs.

Welldoc offers the capability for in-app two-way bidirectional asynchronous chat for 1:1 communication between member and case/care managers that can be initiated by either member or case/care manager. We offer the option of secure asynchronous text or via video chat with one and/or both available to be “turned on” for our customers. In addition, our capability allows for population level messages to be sent to all or a cohort of members by their case/care manager through and in app messaging feature. Case/care managers can monitor incoming messages, respond to chats, initiate video calls, and generate in-app messages from the Welldoc care management portal.

Welldoc is committed to providing a product that can be accessible to members of all populations. We have demonstrated success in many hard to support populations including Medicaid, Medicare and rural (our most recent clinical study). Welldoc's FDA clearance and clinical rigor validates our dedication and commitment to overall public health. Language- All digital coaching through the Welldoc App is at a 5th grade reading level. The Welldoc solution is available in English and neutral Spanish. Welldoc also has the capability to expand our platform to other languages in the future. Device Agnostic The Welldoc application has cross-device operability with over 400 fitness trackers, blood glucose, continuous glucose monitoring, blood pressure, weight scales, labs and pharmacies. Full list available on request. Our platform being device agnostic allows plans to either keep their existing product formularies or when ready to change product formularies have the assurance that any brand selected will integrate with the Welldoc platform. Welldoc is not a strip company so we will not be changing members to a specific brand and we will not be shipping supplies on a monthly basis. Our goal is to reduce DME and Pharmacy spend. Members can keep their current devices and our client will not get any member complaints or service disruptions. SDOH Screening Welldoc strives to combat health iniquities to improve care through utilizing surveys to identify risk for depression, behavioral health needs, and social determinants of health. When members respond, the information is shared with identified client care teams/clinicians for outreach and support. Customer Crae Customer care supports members in both English and Spanish, with a language line available for others languages. Customer care supports account creation, onboarding, understanding app features, and syncing devices. Customer care hours can be modified upon request to satisfy area time zones and other specific needs to accommodate and satisfy program and contractual needs.

Our platform provides condition specific, customizable care plans and pathways for clinical decision support and patient engagement through.

Welldoc has a flexible integration approach Welldoc offerings include the ability to integrate directly into the client's case/care management solutions via Single Sign On (SSO) Integration allowing seamless experience to the care team end users. We support different SSO including SAML, OKTA etc. In addition, Welldoc also offers support via REST API and can easily integrate external APIs such as 'Eligibility API' for real time eligibility checking during patient onboarding and tailor the experience to each patient/user. For clients that do not have APIs we can also accommodate them using secure files exchange via SFTP. With the help of our strategic vendors such as Xealth, Redox or ZeOmega/Jiva (or other Care Management Platform vednors/technologies), we have clients where Welldoc is integrated directly into Care Management/EHR systems allowing write back capabilities via Flowsheet or In Basket messaging. As applicable for those who already have advanced member facing digital assets, we have also developed Software Development Kit (SDK) and API (Application Programming Interface) capabilities that enable the client to build in the Welldoc platform features and functionality directly into your digital assets. Our current set of SDKs are “UI-less” which allows you the flexibility to integrate based on your business needs. One of the major advantages of Welldoc platform is that it is device agnostic. The Welldoc platform leverages patients’ existing Smartphones and integrates with over 400+ connected devices and data sources, including: -Blood glucose meters -Blood pressure monitors -Weight scales -Fitness trackers -Continuous Glucose Monitors (CGM) A device agnostic approach allows individuals to use their existing technology which helps to overcome the inertia and challenges of learning how to use a new set of device(s) as part of starting a new program. We leverage an app based/smartphone driven solution to take advantage of the technology that most patients have with them throughout the day. Our solution also provides connectivity to pharmacies (CVS, Walgreens, Rite-Aid), laboratories (Quest, LabCorp) and patient portal data sources. Data is the key to our personalized coaching experience, and we strive to make it easy for patients to centrally capture and connect their vital health data within the app without having to spend an inordinate amount of time doing daily data entry. A device agnostic approach fosters a relationship between your patients and their providers who can continue to prescribe formulary-covered devices that they and their patients are comfortable with. As your plan optimizes its formulary through coverage changes, Welldoc will work with you to assure connectivity to any new devices. If desired, we have partners that can source additional devices/connection that are not currently part of the program

Our digital-first platform includes artificial intelligence (AI) driven coaching which provides personalized, actionable guidance and longitudinal feedback based on the six dimensions of health: (1) Symptoms, (2) Food, (3) Activity, (4) Psycho-Social, (5) Medications and (6) Labs to help support the patient throughout their chronic care journey. Our program contains over 40,000 FDA approved coaching messages. Our clinical rigor is demonstrated through our FDA cleared product with 11 510k FDA clearances and 90+ peer reviewed publications and journals demonstrating clinical outcome.

For Patients: Our solution is delivered though a mobile app primarily with webs support For Care Teams: Web Portal

Rimidi integrates with a broad portfolio of medical devices, including cellular glucometers, blood pressure cuffs, scales, and pulse oximeters. Rimidi integrates with leading Continuous Glucose Monitors, including FreeStyle Libre, Dexcom, and Eversense. In addition, Rimidi integrates with Adherium's Hailie® Smartinhaler®.

Rimidi's clinical decision support engine drives clinical insights and actions from RPM alerts to guideline and protocol adherence.

Rimidi supports two-way text messaging between the patient and the care team.

Cellular-enabled devices are easy-to-use and do not require the patient to have access to home WiFi or even a smart phone. Rimidi supports texting a patient in their native language, and our clinical monitoring staff can support multilingual needs.

Rimidi has ready-to-use care plan templates that can be customized for use in CCM or other disease management initiatives.

The Rimidi platform most often operates as a Single Sign On (“SSO”) to the provider’s EHR, either through SMART on FHIR or Security Assertion Markup Language (SAML) depending on the EHR. Rimidi has established integrations with most major EHRs, including eClinicalWorks, Epic, OCHIN, Oracle Cerner, Veradigm, athenahealth, and Meditech. Integrations enable Rimidi to pull relevant clinical data into the platform to support gap in care closure and provide robust disease context next to the RPM data. In addition, Rimidi can write back documents (RPM summaries, care plans, etc.) or support HL7 workflows for discrete data write back.

Platform Support: Rimidi has a Client Success team that is dedicated to our customers from program design through implementation that continues for the life of the engagement. Your Client Success Manager (CSM) will come onsite for training and provide additional virtual training sessions as needed. Your CSM and support staff are engaged with you through all aspects of the program, including the set-up of the Rimidi platform, EHR integration, user training, and connecting devices. Care Management Support: Rimidi provides clinical monitoring for many of our clients who want to supplement their current staff or to avoid dedicating in-house staff to new programs considering hiring and staffing challenges. The monitoring staff can include health coaches, registered nurses (RNs), advanced practice providers (APPs), and medical doctors (MDs) depending on the client’s program structure and staffing needs. All monitoring staff carry appropriate state licensures and can support multiple languages as needed. With the efficiency that the Rimidi platform delivers, one clinical user can manage approximately 250 patients.

Pediatric use cases:

None provided

Users:

A user must be over age 18 and can self-identify their cardiometabolic chronic condition(s) for treatment such as diabetes, hypertension, heart failure, etc. If additional eligibility criteria are desired, Welldoc can work with a client to understand their desired eligibility guidelines.

Description:
  • A fully-capitated provider needs a better way to manage their highest risk patients to reduce hospital utilization and improve quality measures. Their centralized disease management team uses Rimidi to monitor weight and pulse oximetry in heart failure patients, CGM or BGM data in diabetes patients, and inhalation quality in their Asthma patients.
  • A physician practice wants to increase practice revenue while taking better care of their patients. They work with Rimidi's care management team to enroll appropriate patients in RPM and CCM programs, and document the clinical and billlable activities within Rimidi.
  • A physician sees a patient with poorly controlled diabetes in the office and is prompted in the Rimidi diabetes view to enroll the patient in remote monitoring and to consider a therapy adjustment to optimize cardiovascular risk reduction. The physician creates a dynamic care plan with each encounter that posts back to their EMR. The physician reviews the patient’s home glucose readings one week after therapy adjustment to ensure adequate control.
  • A patient uses the connected glucometer provided by the practice. She can see whether she is in her target range, and text with her care team in response to her data.
  • A care coordinator monitors a clinic’s patients with diabetes, heart failure or hypertension remotely, addressing alerts configured for the clinic and ensuring that they are meeting clinical guidelines and helping to avoid costly admissions and readmissions. Documentation for RPM and CCM activity is done in the Rimidi app and aggregate for billing purposes.

Differentiators

Differentiators vs EHR Functionality:

Welldoc believes that partnership is the pathway to better management of chronic conditions. Digital health solutions do not work in a silo and data and insights should be integrated into care models to support optimized interventions and clinical decisions. The Welldoc platform includes a digital ecosystem that allows for user coaching and support along with remote monitoring and tracking of patients. In addition, the platform includes integrated patient population management tools, dashboards and analytics.

Backed by Evidence and Clinical Rigor

Welldoc’s platform holds 11 510(k) FDA-Clearances, 90+ clinical publications and posters, and 54 patents for our technology and platform features. Welldoc boasts strong economic and clinical outcomes in randomized, controlled clinical trials and in real-world experience.

Extensive research has shown:

  • Average cost savings of $3,048-$3,252/person with Type 2 diabetes, annually [1]
  • A 1.2-2.0 point mean A1C reduction in individuals living with type 2 diabetes [2,3]
  • 7.4 - 12 mmHg Drop in blood pressure in the first 3 months of use [4]
  • Welldoc currently has a Net Promoter Score (NPS), a standard gauge of digital health customer satisfaction of ~70.1. Individuals engage with the app an average of ~30 times per week [5]

Digital Coaching

Welldoc offers 1:1 asynchronous chat capabilities, two-way messaging, and live video meeting capabilities directly through our application. We enable this functionality for use by our clients’ Care Management teams, allowing them to function most efficiently in data-driven interventions that prioritize the members most in need.

Welldoc is a device-agnostic solution. The Welldoc platform connects to over 400 devices and data sources, including CGM, blood glucose meters, scales, blood pressure devices, fitness trackers, pharmacy and clinical laboratory data. Given Welldoc's device-agnostic approach, preferred supplies/consumable products are dictated and provided by the client. Welldoc can configure the app to support device/supply ordering functionality from within the application if so desired. This also prevents any interference with existing formulary strategies.

Integration into care models

Our Welldoc platform can push notifications, alerts, and reports to care team members and providers within their existing workflows with integration into their EMR or care management platform. Welldoc is capable of sending and receiving data to health systems. Integration details are handled at implementation. Welldoc has existing partnerships with Xealth and Redox to support technical integrations in line with your requirements. We will partner with you to develop and execute an integration approach that can seamlessly fit into your care model and support remote patient monitoring and tracking.

Client Success and Engagement Program

Welldoc also offers full client success and engagement support as part of our offerings to support outreach, awareness building, user activation, and overall engagement with our solution. We can work directly with you to build and execute a comprehensive, multi-channel campaign and communications approach in line with your program objectives and criteria. We have created an approach to support every part of the member journey:

  • Awareness: Building knowledge of total chronic care programs and the advantage of Welldoc's digital health solution.
  • Registration/Activation: Supporting member enrollment activities, onboarding, and application configuration.
  • Engagement: Ensuring members know how to make the best use of Welldoc's digital health solution in line with the total chronic care program; Understanding the data and insights that are critical to longer-term lifestyle changes and optimizing their treatment plan with their care teams; driving deeper engagement by recommending additional features within the application.
  • Graduation/Disenrollment: We will work with you to determine the triggers for graduation or disenrollment. Given the personalization driven by AI, we typically see members utilize our solution throughout their chronic care journey to support a healthy lifestyle.
  • Surveys: Understanding the member experience and receiving feedback at critical points of their journey. We deliver surveys within the app to assess social determinants of health and psychosocial issues, provide resources, and identify pertinent concerns to share with the care team.
  • Data Analytics/Program Success: Welldoc is committed to helping clients and partners optimize their programs, and therefore tracks and reports on program success through actionable data and user intelligence.
  • Customer Service: Welldoc offers telephone and email customer support to help get users activated and get their devices synced. Users can easily access a live agent via phone M-F; 8 am - 6 pm ET.

Livongo

Livongo is typically more expensive since provide human coaches & devices, whereas Welldoc is a digital-first, device agnostic approach. With Welldoc, health plans don’t have to pay for coaching & supplies that members may not be using.

Omada
Omada is typically more expensive because they use human coaches where Welldoc is digital-first, available 24/7/365 and connects user to their own provider. With Welldoc, health plans don’t have to pay for live coaching that members may not be using

Differentiators vs EHR Functionality:

In the sales process, we often hear “Doesn’t Epic (or other EHR) do that?” Rimidi is not built to replace the EHR, but to enhance it by bringing in additional data sets, providing more efficient workflows, and improving the user experience. Rimidi can install and configure its platform within 3-6 weeks which is significantly faster and more cost effective than onsite EHR teams can build and support enhancements to meet clinical needs. Rimidi’s modern technology stack enables rapid cycle innovation and iteration post-go live.

Differentiators vs Competitors:

Rimidi’s cardiometabolic platform is most often compared to other Remote Patient Monitoring solutions, but the platform-approach to RPM allows Rimidi to do so much more. Some key differentiators of the Rimidi platform are: (1) tight integration with leading EHRs via SMART on FHIR, (2) the breadth of the comorbid disease states we cover, (3) our incorporations of patient-reported outcomes and social determinants of health measures and (4) our Clinical Decision Support engine. Rimidi’s CDS cards support almost any insight or intervention a client wants: RPM data alerting and interventions, referral management, lab or medication recommendations, clinical trial identification, and more.
