Socially Determined is a healthcare analytics company based in Washington DC. We are developing the science of the social determinants of health and partnering with our clients to implement community-based intervention programs. We believe that social determinants drive over 50% of our health outcomes and that we can understand those determinants using advanced analytics and data science.
