With strong patient experience results, their practice recently expanded Carium as their digital front door.
  • Health Logx partners with primary care practices to provide fully virtual health and wellness services for patients, on behalf of the practice. They focus on RPM and chronic care management (CCM) programs to track patients in between office visits.
