Leveraging our health IoT platform and access to secure data from 600+ personal health devices, our remote monitoring application doesn’t lock patients and providers into a single device vendor or a few select brands. It offers maximum device flexibility to support remote care for chronic and post-acute conditions. From diabetes and hypertension to COPD and beyond, Validic® Impact remote monitoring manages multiple chronic care programs, concurrently or asynchronously, regardless of condition or population size.

Scale across conditions, populations

Organizations of all sizes can create and scale remote monitoring programs and offer meaningful, continuous and personalized care, across populations and chronic conditions. Welldoc offers reporting packages at the individual patient as well as population level. Patient Level Reporting: Health (Smart Visit) Reports – On a routine/scheduled basis, or as needed between visits, the Smart Visit Report will be shared with our client's clinical team to summarize the patient's current clinical situation, including clinical decision support grounded in evidence based guidelines for our client's clinicians to review suggested self-care behaviors, potential medication alternatives, etc. Patients can also view this report on demand through the patient facing application The Smart Visit Report includes: - 90-day cardiometabolic biometric data summary - Medication plan summary - Trends graphs giving immediate visibility to effectiveness of diabetes management plan and any particular time of day challenges - Clinical and patient self-management summary - Evidence-based clinical decision support recommending potential care plan adjustments based on the data for consideration by the HCP, citing clinical guidelines, and - Optional daily logbook, containing patient data for the past 30 days, including imported and manual cardiometabolic entries, medication adherence, sleep, activity, and member notes adding context to the data inputs. In addition to the Smart Visit Reports, patients receive weekly reports that provide insight into medication adherence, glucose trends, diet and activity summaries and allow them to plan for the upcoming week. A good cardiometabolic condition management program uses effective communication and data collection technologies to provide ease of access to patients and to facilitate better risk stratification via the real-time data collection from physiological measurement devices or patient self-report metrics. From a cost-effectiveness perspective, it is important to provide a range of services to match patient need. Welldoc can either power our customer's Care Team or we can augment our technology platform with our own partner. When we bring our partner to an engagement, through the use of Welldoc’s technology and programs and pairing this with our clinical services team and experience, we have an excellent methodology for targeting higher touch, wrap around services for higher risk patients and lower touch, preventive services for lower severity, but still at-risk groups. Ultimately, we seek to ensure deployment of clinical resources to those patients that are at highest risk of increased emergency department and hospital utilization while working to lower risk in the entire population via preventive coaching and FDA cleared digital programs.

Welldoc offers the capability for in-app two-way bidirectional asynchronous chat for 1:1 communication between member and case/care managers that can be initiated by either member or case/care manager. We offer the option of secure asynchronous text or via video chat with one and/or both available to be “turned on” for our customers. In addition, our capability allows for population level messages to be sent to all or a cohort of members by their case/care manager through and in app messaging feature. Case/care managers can monitor incoming messages, respond to chats, initiate video calls, and generate in-app messages from the Welldoc care management portal.

Welldoc is committed to providing a product that can be accessible to members of all populations. We have demonstrated success in many hard to support populations including Medicaid, Medicare and rural (our most recent clinical study). Welldoc's FDA clearance and clinical rigor validates our dedication and commitment to overall public health. Language- All digital coaching through the Welldoc App is at a 5th grade reading level. The Welldoc solution is available in English and neutral Spanish. Welldoc also has the capability to expand our platform to other languages in the future. Device Agnostic The Welldoc application has cross-device operability with over 400 fitness trackers, blood glucose, continuous glucose monitoring, blood pressure, weight scales, labs and pharmacies. Our platform being device agnostic allows plans to either keep their existing product formularies or when ready to change product formularies have the assurance that any brand selected will integrate with the Welldoc platform. Welldoc is not a strip company so we will not be changing members to a specific brand and we will not be shipping supplies on a monthly basis. Our goal is to reduce DME and Pharmacy spend. Members can keep their current devices and our client will not get any member complaints or service disruptions. SDOH Screening Welldoc strives to combat health iniquities to improve care through utilizing surveys to identify risk for depression, behavioral health needs, and social determinants of health. When members respond, the information is shared with identified client care teams/clinicians for outreach and support. Customer Crae Customer care supports members in both English and Spanish, with a language line available for others languages. Customer care supports account creation, onboarding, understanding app features, and syncing devices. Customer care hours can be modified upon request to satisfy area time zones and other specific needs to accommodate and satisfy program and contractual needs.

Welldoc has a flexible integration approach Welldoc offerings include the ability to integrate directly into the client's case/care management solutions via Single Sign On (SSO) Integration allowing seamless experience to the care team end users. We support different SSO including SAML, OKTA etc. In addition, Welldoc also offers support via REST API and can easily integrate external APIs such as 'Eligibility API' for real time eligibility checking during patient onboarding and tailor the experience to each patient/user. For clients that do not have APIs we can also accommodate them using secure files exchange via SFTP. With the help of our strategic vendors such as Xealth, Redox or ZeOmega/Jiva (or other Care Management Platform vednors/technologies), we have clients where Welldoc is integrated directly into Care Management/EHR systems allowing write back capabilities via Flowsheet or In Basket messaging. As applicable for those who already have advanced member facing digital assets, we have also developed Software Development Kit (SDK) and API (Application Programming Interface) capabilities that enable the client to build in the Welldoc platform features and functionality directly into your digital assets. Our current set of SDKs are “UI-less” which allows you the flexibility to integrate based on your business needs. One of the major advantages of Welldoc platform is that it is device agnostic. The Welldoc platform leverages patients’ existing Smartphones and integrates with over 400+ connected devices and data sources, including: -Blood glucose meters -Blood pressure monitors -Weight scales -Fitness trackers -Continuous Glucose Monitors (CGM) A device agnostic approach allows individuals to use their existing technology which helps to overcome the inertia and challenges of learning how to use a new set of device(s) as part of starting a new program. We leverage an app based/smartphone driven solution to take advantage of the technology that most patients have with them throughout the day. Our solution also provides connectivity to pharmacies (CVS, Walgreens, Rite-Aid), laboratories (Quest, LabCorp) and patient portal data sources. Data is the key to our personalized coaching experience, and we strive to make it easy for patients to centrally capture and connect their vital health data within the app without having to spend an inordinate amount of time doing daily data entry. A device agnostic approach fosters a relationship between your patients and their providers who can continue to prescribe formulary-covered devices that they and their patients are comfortable with. As your plan optimizes its formulary through coverage changes, Welldoc will work with you to assure connectivity to any new devices. Welldoc is FDA approved for individuals with T1 or T2 on insulin. Medications (including insulin) can be tracked. Advanced insulin capabilities include basal and bolus dose titration program and Bolus Calculator. Insulin pump education and support has been built into Welldoc and is currently under FDA approval.

Validic's remote care application supports more than 100 devices to allow patients to share biometric data from outside the hospital or clinic. It is extended to patients in two ways. First, it allows for a BYOD smartphone offering, where the patient can download our HealthBridge™ app and complete enrollment and device pairing on their personal device. Peripheral devices such as scales and blood pressure cuffs are provided by their care team or purchased by the patient online or in stores. The application also allows for the shipping of a pre-configured, locked-down tablet (kit), where all peripherals can be pre-paired and it "just works" out of the box. Whether provisioned from the BYOD or "kit-based" model, the data seamlessly flow from the patient's device and into the RPM program, where it is evaluated against programmed rules, is able to escalate alerts directly to providers or nursing queues, and is viewable and actionable within the EHR.

The client configures Validic's program defaults, with customizeable rules at the program and individual patient levels. The data we receive from the patient's devices are evaluated against the program rules and the result of those evaluations can be alerts to providers, messages to specified individual or shared inboxes, follow-up surveys for more information, or messages sent directly to the patient with instructions on what actions they should take. Our clients build their programs with the appropriate measurement goals and thresholds to enable them to have awareness of any concerning trends in a patient's condition.

Validic's remote care application also includes asynchronous, conversational chat capability. They are used in two ways. First, to evaluate data received from a patient's device. For example, if a blood pressure reading is above the threshold set by the care team in that patient's program, the team can trigger a follow-up conversation to gather additional information from the patient such as worsening symptoms or medication adherence. Second, care teams can send chat conversations regularly (daily, weekly, etc) as a way to check in with the patient about their condition, progress in the program, or to gain other insights. The care team can build rules that evaluate the chat conversation responses and deliver alerts and escalations in a variety of ways, such as to a configured EHR message inbox. For example, an answer to a question of "are you feeling better/same/worse today" with an answer of "worse" could trigger a message for follow up.

Our patient-facing solutions can be offered on a pre-configured "zero setup" tablet, where the patient's peripheral devices are pre-paired and ready to go out of the box. The patient simply types in a PIN code and leaves the tablet plugged in/charged and in close proximity to the peripheral devices when taking readings. There are no other actions for the patient to take other than using their blood pressure cuff, scale, etc to take readings. For users who may already have their own glucometers, scales, or blood pressure cuffs, we support an expansive library of devices and continue to add more to our platform annually. This allows many patients to use the medical device with which they are already comfortable. Third, for patients who are comfortable using their own smartphone/tablet, we support Android and iOS through our HealthBridge™ native apps. After completing a simple, guided self-enrollment process, the patient is directed to pair their device, and readings seamlessly flow through the app as long as it's running in the background. The patient isn't required to open the app or take any additional actions beyond recording measurements using their peripheral devices in proximity of the phone where the HealthBridge™ app is installed. Finally, the HealthBridge™ app runs in English and Spanish.

Starting with industry-standards-based Blueprints, our implementation team guides each customer through the creation and customization of care plans for each program into which that the customer is planning to enroll patients. These goals, rules, and alerts allow our customers to build highly specialized care plans that enable the care team to be alerted and informed regarding patients who need to be engaged, while giving them confidence on patients who are performing well in the program. Validic's remote care application allows for each program to have a default configuration that will apply to each enrolled patient, while also enabling the care team to modify thresholds and alerts on a patient-by-patient basis, to give as much flexibility as is needed to manage care.

Validic's digital health and remote care platform is integrated with the Cerner and Epic EHRs, and NextGen Video Visit solution. Our integration includes: - Seamless integration of the provider clinical experience within the EHR via an embedded SMART on FHIR application. This includes SSO so there is never a need to have a second password. Providers work directly in the EHR and never have to "launch out" to a second application. - Enrollment of a patient into a program happens directly within the integrated experience. The care team can see which programs a patient is already enrolled into and enroll them into additional programs, all within the EHR embedded workflow. Data needed for enrollment is automatically read from the EHR, avoiding the need for the care team to manually type in demographic information. - The ability to generate inbox/inbasket messages directly into the EHR's native messaging system. This makes the information generated by remote care programs live alongside other messages that are being sent to the care team, including shared inboxes/inbaskets, to facilitate workflows where the care team is flexible throughout the day/week. - Writing the discrete result values received from the patients devices directly into the EHR flowsheet, so that it is evaluated by Validic's RPM rules within the program and also becomes part of the patient's medical record, to be used by other processes within the EHR as well as any downstream analytics, population health, HIEs, or other systems that depend on the EHR chart being comprehensive.

For higher-acuity programs or programs for individuals with lower technical abilities, Validic can provide everything from simple ship-and-forget clinical device fulfillment and IT setup and configuration to reverse logistics, friendly and compassionate 1-800 patient-facing technical support, and outbound onboarding calls to support a patient’s first experience with the programs. Staff training is part of our customer onboarding/go-live processes.

Use Cases

Description:

New York City Health and Hospitals (NYC H+H) with 273 total participants on BlueStar demonstrated reduction in A1c with an average six-month reduction in A1c (1.1 vs. 0.2; -0.9, p<0.01). There were high levels of engagement with more than half of the participants used the app once a week; recording blood glucose and medications were the most frequently used features (82% and 62% respectively). Of BlueStar participants, 40% were Spanish-speaking and received the Spanish-version of BlueStar. All English- and Spanish-speaking participants found BlueStar easy to use and understand.

Montana Rural Case Study

The state of Montana provided Welldoc’s digital health solution for diabetes, FDA-cleared BlueStar®, to eligible individuals with type 1 or type 2 diabetes* as part of its effort to scale DSMES services. The program sought to identify whether a digital tool could expand access to and participation in these services. It was an exploration of innovativeways to eliminate barriers for individuals with diabetes from participating in programs and improve retention as well as health outcomes. 

Results:

Glycemic Outcomes - An Average improvement of 47 mg/dL amongst engaged users

Blood Pressure Outcomes - An average reduction of 10 mmHg amongst those who showed reduction

Weight Outcomes - An average of 7% reduction of weigth from baseline amongst those who lost weight

An integrated digital health solution can support health systems and health plans in expanding access and extending existing care programs. Digital Coaching

Welldoc offers 1:1 asynchronous chat capabilities, two-way messaging, and live video meeting capabilities directly through our application. We enable this functionality for use by our clients’ Care Management teams, allowing them to function most efficiently in data-driven interventions that prioritize the members most in need.

Welldoc is a device-agnostic solution. The Welldoc platform connects to over 400 devices and data sources, including CGM, blood glucose meters, scales, blood pressure devices, fitness trackers, pharmacy and clinical laboratory data. Given Welldoc's device-agnostic approach, preferred supplies/consumable products are dictated and provided by the client. Welldoc can configure the app to support device/supply ordering functionality from within the application if so desired. This also prevents any interference with existing formulary strategies.

  • Graduation/Disenrollment: We will work with you to determine the triggers for graduation or disenrollment. Given the personalization driven by AI, we typically see members utilize our solution throughout their chronic care journey to support a healthy lifestyle.
  • Surveys: Understanding the member experience and receiving feedback at critical points of their journey. We deliver surveys within the app to assess social determinants of health and psychosocial issues, provide resources, and identify pertinent concerns to share with the care team.
  • Data Analytics/Program Success: Welldoc is committed to helping clients and partners optimize their programs, and therefore tracks and reports on program success through actionable data and user intelligence.
  • Customer Service: Welldoc offers telephone and email customer support to help get users activated and get their devices synced. Users can easily access a live agent via phone M-F; 8 am - 6 pm ET.
