Leveraging our health IoT platform and access to secure data from 540+ personal health devices, our remote monitoring application doesn’t lock patients and providers into a single device vendor or a few select brands. It offers maximum device flexibility to support remote care for chronic and post-acute conditions. From diabetes and hypertension to COPD and beyond, Validic® Impact remote monitoring manages multiple chronic care programs, concurrently or asynchronously, regardless of condition or population size.

Organizations of all sizes can create and scale remote monitoring programs and offer meaningful, continuous and personalized care, across populations and chronic conditions. Standalone, disease-specific platforms are not necessary or economical. Clinicians can enroll and monitor patients in an RPM program directly from their chart, launch our application from In-Basket messages, and streamline workflow by eliminating unnecessary clicks, toggles and uploads. 

Ease the burden on clinicians

Because we know healthcare providers’ time is better spent with patients and not on administration, we designed our remote monitoring application to fit seamlessly into the systems they already know and use. Organizations don’t need to hire additional staff or allocate extra resources to RPM programs. Clinicians can view and manage critical data and notifications within the clinical workflow and view historical trends directly in the patient’s health record. They don’t need to be data scientists and will notice a reduction in redundant and inefficient manual tasks, ensuring they spend more time on what matters most - direct patient care.

OnCare360 collects a wide range of data, including clinical information such as vital signs, diagnoses, medications, and post-discharge details; care management data like time spent on coordination and care plan updates; and patient-generated health data from RPM and RTM devices. It also gathers social determinants of health (SDOH) data, including housing, transportation, and other socioeconomic factors, along with behavioral health metrics such as mental health screenings and care plans. Preventive care data, such as Health Risk Assessments (HRA), Fall Risk Assessments (FRA), and Advance Care Planning (ACP) directives, are also collected, as well as engagement data from patient portals, communication logs, and reminders. Additionally, OnCare360 tracks billing and compliance data, including CPT codes and audit-ready documentation.

Validic's remote care application supports more than 100 devices to allow patients to share biometric data from outside the hospital or clinic. It is extended to patients in two ways. First, it allows for a BYOD smartphone offering, where the patient can download our HealthBridge™ app and complete enrollment and device pairing on their personal device. Peripheral devices such as scales and blood pressure cuffs are provided by their care team or purchased by the patient online or in stores. The application also allows for the shipping of a pre-configured, locked-down tablet (kit), where all peripherals can be pre-paired and it "just works" out of the box. Whether provisioned from the BYOD or "kit-based" model, the data seamlessly flow from the patient's device and into the RPM program, where it is evaluated against programmed rules, is able to escalate alerts directly to providers or nursing queues, and is viewable and actionable within the EHR.

The client configures Validic's program defaults, with customizeable rules at the program and individual patient levels. The data we receive from the patient's devices are evaluated against the program rules and the result of those evaluations can be alerts to providers, messages to specified individual or shared inboxes, follow-up surveys for more information, or messages sent directly to the patient with instructions on what actions they should take. Our clients build their programs with the appropriate measurement goals and thresholds to enable them to have awareness of any concerning trends in a patient's condition.

Validic's remote care application also includes asynchronous, conversational chat capability. They are used in two ways. First, to evaluate data received from a patient's device. For example, if a blood pressure reading is above the threshold set by the care team in that patient's program, the team can trigger a follow-up conversation to gather additional information from the patient such as worsening symptoms or medication adherence. Second, care teams can send chat conversations regularly (daily, weekly, etc) as a way to check in with the patient about their condition, progress in the program, or to gain other insights. The care team can build rules that evaluate the chat conversation responses and deliver alerts and escalations in a variety of ways, such as to a configured EHR message inbox. For example, an answer to a question of "are you feeling better/same/worse today" with an answer of "worse" could trigger a message for follow up.

Our patient-facing solutions can be offered on a pre-configured "zero setup" tablet, where the patient's peripheral devices are pre-paired and ready to go out of the box. The patient simply types in a PIN code and leaves the tablet plugged in/charged and in close proximity to the peripheral devices when taking readings. There are no other actions for the patient to take other than using their blood pressure cuff, scale, etc to take readings. For users who may already have their own glucometers, scales, or blood pressure cuffs, we support an expansive library of devices and continue to add more to our platform annually. This allows many patients to use the medical device with which they are already comfortable. Third, for patients who are comfortable using their own smartphone/tablet, we support Android and iOS through our HealthBridge™ native apps. After completing a simple, guided self-enrollment process, the patient is directed to pair their device, and readings seamlessly flow through the app as long as it's running in the background. The patient isn't required to open the app or take any additional actions beyond recording measurements using their peripheral devices in proximity of the phone where the HealthBridge™ app is installed. Finally, the HealthBridge™ app runs in English and Spanish.

Starting with industry-standards-based templates, our implementation team guides each customer through the creation and customization of care plans for each program into which that the customer is planning to enroll patients. These goals, rules, and alerts allow our customers to build highly specialized care plans that enable the care team to be alerted and informed regarding patients who need to be engaged, while giving them confidence on patients who are performing well in the program. Validic's remote care application allows for each program to have a default configuration that will apply to each enrolled patient, while also enabling the care team to modify thresholds and alerts on a patient-by-patient basis, to give as much flexibility as is needed to manage care.

Validic's digital health and remote care platform is integrated with the Cerner and Epic EHRs, and NextGen Video Visit solution. Our integration includes: - Seamless integration of the provider clinical experience within the EHR via an embedded SMART on FHIR application. This includes SSO so there is never a need to have a second password. Providers work directly in the EHR and never have to "launch out" to a second application. - Enrollment of a patient into a program happens directly within the integrated experience. The care team can see which programs a patient is already enrolled into and enroll them into additional programs, all within the EHR embedded workflow. Data needed for enrollment is automatically read from the EHR, avoiding the need for the care team to manually type in demographic information. - The ability to generate inbox/inbasket messages directly into the EHR's native messaging system. This makes the information generated by remote care programs live alongside other messages that are being sent to the care team, including shared inboxes/inbaskets, to facilitate workflows where the care team is flexible throughout the day/week. - Writing the discrete result values received from the patients devices directly into the EHR flowsheet, so that it is evaluated by Validic's RPM rules within the program and also becomes part of the patient's medical record, to be used by other processes within the EHR as well as any downstream analytics, population health, HIEs, or other systems that depend on the EHR chart being comprehensive.

For higher-acuity programs or programs for individuals with lower technical abilities, Validic can provide everything from simple ship-and-forget clinical device fulfillment and IT setup and configuration to reverse logistics, friendly and compassionate 1-800 patient-facing technical support, and outbound onboarding calls to support a patient’s first experience with the programs. Staff training is part of our customer onboarding/go-live processes.
