DESCRIPTION

The Medocity Digital Care Platform is a comprehensive, integrated ecosystem that is designed via a whole person approach to connect patients to their care teams in real time, wherever they may be. It enables continuous care and connectivity when the patient is not in the doctor’s office or clinic, including telehealth, secure messaging, remote patient monitoring of vital signs, symptoms and social determinants of health. It also provides patients with AI-driven important and patient-friendly information they need to understand and manage their conditions, in their language of choice. The Platform provides care teams with a real-time view of a patient’s symptoms, vital signs, and adherence to medications and care plans, and allows providers, patients and caregivers with secure means to communicate via secure text messaging for asynchronous communication and a powerful telehealth (phone and televideo) for synchronous communication. This enables earlier detection and more pro-active intervention, giving our clients the solution to reduce costs while improving outcomes and patient and provider satisfaction.
