Rimidi’s cardiometabolic platform supports disease management of some of the most prevalent and costly chronic conditions. The platform provides access to the tools required to manage multiple disease states – Diabetes, Cardiovascular Disease/Hypertension, Heart Failure, Nonalcoholic Fatty Liver Disease (NAFLD) and Nonalcoholic Steatohepatitis (NASH), and Obesity. The platform consolidates relevant clinical data from the EMR for each disease state with patient-generated health data from connected devices or surveys, and layers clinical decision support that can be mapped against national or practice guidelines. Glooko® Web App for Professionals is a comprehensive, in-clinic and remote patient management platform that allows health care providers to view and analyze diabetes data from one location. With Glooko’s free mobile app, people living with diabetes can easily sync and view their diabetes health data, track medication, log food and exercise, and connect remotely to your clinic.

Glooko offers customizable Care Programs, a structured journey, either assigned by the provider or the patient, that allows the patient to progress through a series of objectives or goals that track and/or educate them on a predesignated topic. Care Programs can revolve around condition, medication, duration of diagnosis, or any other scenario that could benefit from more frequent touchpoints than a regular checkup.

With the broadest range of device compatibility on the market, the Glooko® Mobile App makes it easy to sync with whatever diabetes device or health and fitness app patients are using.

Glooko syncs with over 200 devices, including popular blood glucose meters, continuous glucose monitors, insulin pumps, insulin smart pens, and fitness/biowearable devices, such as Apple Health and Fitbit.

Glooko’s EHR Integration Suite supports the ability to integrate data with EHR vendors and beyond. Our technology is backed by a comprehensive suite of HL7 v2 supported workflows as well as an expansive set of JSON formatted Rest API endpoints. View and analyze patients’ diabetes data synced remotely through the Glooko® Mobile App or in-clinic via the Glooko® Transmitter. Identify at-risk trending patients, access in-clinic and remotely shared data on demand as interactive reports, and target specific patients or groups as needed.

Use Cases

Description:
  • A physician sees a patient with poorly controlled diabetes in the office and is prompted in the Rimidi diabetes view to enroll the patient in remote monitoring and to consider a therapy adjustment to optimize cardiovascular risk reduction. The physician creates a dynamic care plan with each encounter that posts back to their EMR. The physician reviews the patient’s home glucose readings one week after therapy adjustment to ensure adequate control.
  • A patient uses the connected glucometer provided by the practice. She can see whether she is in her target range, receive messages from her care team in response to her data, and access the diabetes educational course recommended to her on her home device. She feels more connected and in control of her diabetes.
  • A care coordinator monitors a clinic’s patients with diabetes, heart failure or hypertension remotely, addressing alerts configured for the clinic and ensuring that they are meeting clinical guidelines and helping to avoid costly admissions and readmissions. Documentation for RPM and CCM activity is done in the Rimidi app and aggregate for billing purposes.
  • A primary care physician scans a patient population in the Rimidi app and sees several patients with cardiometabolic risk factors that are at risk for Nonalcoholic Fatty Liver Disease. The physician receives guideline-based, clinical decision support to guide the patients to the appropriate level of care—whether that’s a specialist referral, bloodwork, or imaging.
  • A clinic uses Rimidi to send depression screening questionnaires before all annual physician exams for their patients with heart failure and diabetes. Patients can complete the questionnaire on any device and Rimidi posts the responses back to the EMR as well as visualizing them within the Rimidi app.
