PatientConnect Complete is the most comprehensive solution for patients, supported by the HRS standard kit including the HRS tablet accompanied with a Weight Scale, Blood Pressue monitor, and Pulse Ox. Clients have the option to opt for additional peripherals should they see fit, with the option to choose from a thermometer, glucometer. Additional peripherals are available at additional cost. PatientConnect Complete units are ready to go out of the box – including Bluetooth pairing, charged and ready for use by patient.

Biometric Devices Supported:
All HRS peripheral devices are FDA approved and connected through Bluetooth to our tablets. Peripheral devices automatically push patient readings into the clinical portal for analysis. Bluetooth devices allow the biometric readings to automatically load into the tablet and will flow over to the Clinician portal. Patients can also manually enter their readings.

PatientConnect Complete has the ability for multi-channel videoconferencing, phone calls, and messaging to clinicians and family memebers.

Product Capabilities

HRS empowers patients to share biometric and health data in real time with their care team. This ranges from the HRS Kit experience, which features an HRS tablet and Bluetooth-enabled peripherals like a scale, pulse oximeter, blood pressure monitor, glucometer, and thermometer. Additionally, cellular-enabled devices (scale, BP monitor, pulse oximeter, glucometer) are available, and for higher acuity care settings, continuous monitoring supports site-of-care transitions and real-time tracking (see other HRS products for more information). HRS also facilitates seamless communication between the patient and clinician through multi-user video calling, bidirectional voice, and text messaging feature. The platform further enhances patient care with educational content, symptom surveys, and medication reminders.

HRS provides a robust suite of analytics to support program success and optimize operations. The platform features patient vital sign algorithms for risk stratification, allowing organizations to set custom alert parameters. Based on these parameters, the system automatically stratifies patients by acuity, from highest to lowest risk. For patients who fall outside of set thresholds or are deemed higher risk, the system generates alerts for clinicians or displays a visual alert within the portal.

HRS' RPM solutions supports on-demand bidirectional voice, video, and messaging – as well as multi-user video calling.

HRS provides a comprehensive offering, built at a third grade reading level, with large iconography, to support the needs of patients of a wide range of literacy levels and ensure that we increase access to care. HRS also supports 16 different languages as part of our solution.

Once digital tools are ordered, Xealth can manage all elements of delivery and enrollment, either by sending them directly via email or SMS or via the patient portal (stock or custom-built). As patients use these programs, Xealth tracks engagement and can send reminder notifications.

Tthe Xealth platform has successfully deployed a wide variety of connected device solutions within its digital health formulary across the client base. Remote Patient Monitoring solutions deployed include both broad-based RPM such as VitalTech and Harmonize, and also focused RPM solutions such as Babyscripts, Force Therapeutics, Glooko, and Rimidi. Xealth enhances the native capabilities of each of these RPM solutions through its unique integration within the EHR, allowing for the ordering and monitoring of the RPM solution based on configurable use cases. Xealth’s Digital Command Center provides engagement data for digital tools and content deployed through Xealth. Health systems can see how many patients open an email/text message, click on a card in digital care, and watch or view content. This allows the health system to see if content is being used by patients. Health systems can change the time of deployment of a solution (send sooner or later) and/or send reminders, when there is a lower level of engagement than expected. Xealth’s monitor tab, which is integrated into Epic, displays third party application dashboards within Epic, providing a seamless experience for the clinician. The clinician is also able to see engagement data for content, such as videos/pdfs.
