Description

Product Description:

Remote patient monitoring (RPM) is a data tracking telehealth tool that enables providers to monitor health information for patients with acute or chronic conditions on an ongoing basis. Connected devices capture patient medical data such as blood pressure and blood glucose, empowering them to take charge of their health and wellness. Providers then monitor their patients’ health with daily updated data, substantially improving the quality of care.

Whether at home, work, or even vacation, your patients can regularly, easily and privately report their health to you, closing the gap between office visits. With remote patient monitoring software, you’ll have everything you need to help your patients, staff, and practice thrive in today’s digital age.

About CoachCare:

CoachCare powers over 3,000 health systems and independent practices. We enable these organizations to add hundreds of thousands in revenue and improve patient outcomes in 2 weeks without adding staff or chasing new patients. We do this with our award-winning technology platform, which gives practices the tools to provide new Remote Patient Monitoring (RPM) programs. At CoachCare, we take our RPM technology platform beyond the software to provide you with everything you need to drive significant revenue and improved outcomes in 2 weeks. CoachCare provides the software and at-home monitoring devices to deliver RPM, and gives you the option to use either your staff or our staff to perform the monitoring and billing aspects of RPM. We also give all our clients access to our certified billing experts who ensure you deliver an impactful program and get paid on your claims. This is demonstrated by our clients' significant outcomes improvements (e.g., 86% more weight loss) and 95%+ claims success rate.

Differentiators

Differentiators vs EHR Functionality:

Differentiators vs Competitors:

Differentiators vs EHR Functionality:

Cadence is capable of two-way integration with Cerner, Epic, Athena, and other EMRs. Patient vitals and symptoms data captured by Cadence will be accessible via dashboards, notes, and reports in the Cadence platform and in our partners' physicians' EMRs.

Differentiators vs Competitors:

Cadence solves the three major challenges that have plagued RPM historically for our health system partners:

  1. RPM creates too much work for overburdened clinicians
  2. Narrow point solutions create the need for too many vendors
  3. Financial model lacks a clear return on investment

Cadence’s solution includes:

  1. Embedded Clinical Team. Cadence’s remote patient monitoring and responsive virtual care program is supported by an NP-led, embedded clinical team capable of administering care to their partners’ patients in accordance with disease state-specific protocols approved by each partner’s clinicians as part of the implementation process. These clinical protocols help PCPs intervene as necessary so that together, the Cadence Care team and the patient’s PCP, can make adjustments to the care plan for their chronic illness as needed. This results in more proactive, preventive patient care and a reduction in clinicians’ workload. Historically, the Cadence Care team has been able to resolve 99% of patient interactions without the need for escalation to the partner clinician.
  2. Flexible, End-to-End Technology. Cadence is built on top of a flexible technology infrastructure that adapts to support our partners’ remote monitoring and virtual care needs, including the ability to integrate directly into a partner’s EMR and incorporate new devices, use cases, levels of acuity, and care pathways. Cadence’s seamless technology and patient-centric user experience is driving industry-leading engagement today, with over 88% of patients sharing vitals at least 16 days per month.
  3. Low-Risk Financial Model. Cadence offers a low-risk financial model with fees tied to performance and actual patient utilization. Cadence makes the upfront capital investment to support the implementation of the remote monitoring program, including approved devices, deployment configuration, enrollment staff, clinical staff, technical support staff, and logistics. In both fee-for-service and value-based arrangements, Cadence only invoices partners for services rendered to the patients.
