In use by hospital systems, state agencies, insurance plans, and self-funded employers, HabitNu DPP is an evidenced-based personalized lifestyle change program focused on preventing type 2 diabetes. HabitNu DPP is modeled after CDC’s National Diabetes Prevention Program and can be delivered in-person or online (virtually) in a small group setting under the guidance of a health coach. Lasting one year, HabitNu DPP provides participants with lifestyle modification support around healthy eating, increased physical activity, and stress reduction. The curriculum delivers a “lifestyle modification” approach in which participants receive personal coaching, education, and motivational counseling to help them lose weight by improving their diet and increasing their exercise.
