Cadence’s remote patient monitoring and responsive virtual care program is supported by an NP-led, embedded clinical team capable of administering care to their partners’ patients in accordance with disease state-specific protocols approved by each partner’s clinicians as part of the implementation process. These clinical protocols help PCPs intervene as necessary so that together, the Cadence Care team and the patient’s PCP, can make adjustments to the care plan for their chronic illness as needed. This results in more proactive, preventive patient care and a reduction in clinicians’ workload. Historically, the Cadence Care team has been able to resolve 99% of patient interactions without the need for escalation to the partner clinician. 
2. Flexible, End-to-End Technology. Cadence is built on top of a flexible technology infrastructure that adapts to support our partners’ remote monitoring and virtual care needs, including the ability to integrate directly into a partner’s EMR and incorporate new devices, use cases, levels of acuity, and care pathways.
