CancerNAV is an application used to support patient navigators as they manage the treatment path for their oncology patient population through the health system. Based on the Cleveland Clinic Taussig Cancer Center’s patient navigation software, CancerNAV offers a seamless way for patient navigators to identify, track, and manage barriers to care and interventions, and provide follow-up for the oncology patient population in real-time. CancerNAV is a Smart on FHIR app that fully integrates with the EHR.

Today, EMRs focus on clinical treatment and pose challenges with tracking the patient journey. Improved efficiency using CancerNAV allows navigators to serve significantly more patients, facilitate timely access to care and encourage patient engagement.
