Product Capabilities

Through our Proprietary Smart Hubs, mobile applications, wearables, and commercially available monitoring devices, patients can share their biometrics and other health data with their care team from outside the hospital or clinic.

Based on the patient-specific, physician-approved treatment plans, the platform automatically notifies the care team when a patient's readings fall outside a predetermined range. The care team can immediately follow up with the patient to investigate the situation and escalate to the provider or emergency services as needed.

Patients can communicate synchronously with their care team during regular business hours through phone, text, secure messaging, or directly from the patient app or through the Interactive Video Smart Hub. They can use these same modes to send messages to their care team outside of regular business hours. RCP does not currently offer a 24/7 monitoring service.

Patients can be engaged through phone and text modes if they have limited technical skills. Multi-lingual care teams are available through the platform.

Care plans are generated from a templated-based system. Care plans are built on a case-by-case basis with conditions-specific responses, approved by the physician, and used to guide patient engagement during calls with their nurse coach.

The RCP platform is built and supported by an internal team of experienced software designers and engineers. Specific, custom integration is available. Industry standards (such as HL7 and FHRR) are used to facilitate integration and interoperability.

RCP maintains a library of both text and video-based training for patients and staff. Our technical and clinical support hotlines are staffed during regular business hours.

Nurse/Health coaches regularly review patient data and are required to speak to every patient monthly. They review each patient's health goals (as specified in the care plan). They can also push specific educational articles and documents to the patient via text or through the patient's Smart Hub.

OnCare360 collects a wide range of data, including clinical information such as vital signs, diagnoses, medications, and post-discharge details; care management data like time spent on coordination and care plan updates; and patient-generated health data from RPM and RTM devices. It also gathers social determinants of health (SDOH) data, including housing, transportation, and other socioeconomic factors, along with behavioral health metrics such as mental health screenings and care plans. Preventive care data, such as Health Risk Assessments (HRA), Fall Risk Assessments (FRA), and Advance Care Planning (ACP) directives, are also collected, as well as engagement data from patient portals, communication logs, and reminders. Additionally, OnCare360 tracks billing and compliance data, including CPT codes and audit-ready documentation.

These analytics include real-time dashboards for tracking patient outcomes, care plan adherence, and program performance across RPM, CCM, TCM, and other CMS programs.
