It starts with a referral from the physician to a health coach. Kannact’s board-certified, culturally-competent health coach then establishes a one-on-one relationship with the patient. They craft a personalized behavior-change plan around the patient’s disease management regimen, accounting for the patient’s unique situation and SDoH and taking responsibility for their unique gaps in care. The Kannact platform provides our coaches with a full RPM solution, with devices, lab results, and medication adherence via realtime Rx data from thousands of pharmacies. Omnichannel engagement tools allow coaches to truly meet patients where they are. The result is a coach-led, patient-loved, equitable solution that requires minimal change to provider workflow, adding the engagement and monitoring layer that was missing and leaving physicians to focus on what they do best: treating the patient in front of them.
