Our algorithm and care teams suggests content tailored to individual patients based on their care plan, previous content engagement, and care journey.

We deliver multimodal care and can be flexible based on patient and caregiver needs. For patients that lack access to quality broadband we are able to call, SMS, and send materials in the mail. Our content library includes video, audio, and written content.

pCare delivers patient education from the leading content providers (Krames, Healthwise, WedMD, Health Nuts, Elsevier) whose content is kept up to date and reviewed.

Patients can interact with the pCare system via a pillow speaker, tablet, or personal device paired to our system. On Screen Displays alert the patient to view education in preparation for discharge as well as present surveys patients complete in order to give staff a pulse check on an admission. Patients can further interact with the system by answering learning verification questions, placing service quests, ordering meds to beds, ordering meals, and managing room temperature and lighting controls.

pCare CareFlows are triggered by both EHR data and interactions with our system to guide the patient during an admission. Available CareFlows include Discharge Planning, Falls Prevention, Infection Control, and Pain Management.

Navigating the system is as easy as using a television remote and it can be done via a person device. The content presented is from validated patient education publishers whose content is not only scientifically accurate but created in accordance with health literacy design priciples. Via the personalized patient portal accesssible on the in-room TV, patient have access to a wide variety of education that can be automatically prescribed via data from the EHR as well as accessed on demand. All education transactions are documented back into the EHR. Through extensive interviews with caregivers, we know that getting to a doctor’s visit is oftentimes the most disruptive part of the care journey. Not only does this create more friction and disorientation for beneficiaries, but for many caregivers, getting their loved one to a doctor is simply not an option due to geographic location or economic status. Virtual care allows Remo to mitigate this pain point for dyads in rural communities or communities with a high Area Deprivation Index — bringing the village of care directly to them. We seek to deeply understand the communities we serve and pinpoint where our service is most needed. We do this by closely examining ADI, dual-eligibility and prevalence of beneficiaries receiving a low-income subsidy in any given county to ensure we are servicing areas with greatest need.

To assess dyads holistically, we screen for social determinants of health (e.g. economic factors, education/health care access and quality, neighborhood and built environment, social/community context) to identify barriers to care. These inputs are integrated into our proprietary risk scores, enabling our IDT to build out person-centric care plans that address holistic needs of the dyad and proactively remove barriers to care. Our robust analytics infrastructure enables reporting on race, ethnicity, language, gender, sexual orientation and disability status. This foundation is how we will examine intervention effectiveness and outcomes across each of these groups, laying the foundation for how we’ll improve health equity in dementia care in key areas of need.
