Welldoc’s comprehensive chronic care platform provides multi-condition support across diabetes, hypertension, and heart failure with integrated behavioral health support. Wellness, nutrition counseling, and lifestyle management are all core to our solution. Welldoc’s flagship product, BlueStar®*, an FDA-cleared digital health solution, guides individuals through the complicated journey of living with diabetes by enabling them to self-manage their care while enhancing connections to their healthcare team.

Use Cases

Description:

We approach populations with a personalized roadmap of rewards tailored to each step of a complex or preventive care journey and/or preventive care. Complex care examples: high-risk maternal health, diabetes management, addiction, Asthma Medication Ratio, timely postpartum care, and nearly any other clinical intervention. Preventive care examples: Adult Annual Well Visits, breast cancer screenings, cervical cancer screenings, HbA1c screening, childhood lead Screening, Well Visits – Baby/Child/Teen, colon cancer screenings.

Leveraging our proprietary CARE Platform, we design, build, and manage custom incentive programs delivered to the consumer on the TheraPay Rewards app. Each campaign includes (1) Behavioral & Statistical Modeling (2) Customized Engagement & Communication and (3) ROI Optimization & Validation.

Members receive a no-fee debit card so rewards can be paid at the time task completion.

Pediatric use cases:

Complex care examples: high-risk maternal health, Asthma Medication Ratio, timely postpartum care, and nearly any other clinical intervention. Preventive care examples: childhood lead Screening, Well Visits – Baby/Child/Teen.

Description:
  1. New York City Health and Hospitals (NYC H+H) with 273 total participants on BlueStar demonstrated reduction in A1c with an average six-month reduction in A1c (1.1 vs. 0.2; -0.9, p<0.01). There were high levels of engagement with more than half of the participants used the app once a week; recording blood glucose and medications were the most frequently used features (82% and 62% respectively). Of BlueStar participants, 40% were Spanish-speaking and received the Spanish-version of BlueStar. All English- and Spanish-speaking participants found BlueStar easy to use and understand.
  2. A top 3 nationally-recognized payor, Healthagen, with Welldoc and LifeScan conducted a study of 291 total participants. Clinically significant reductions in A1c & blood glucose were demonstrated including an average 0.54 reduction in A1c and 1.83 average A1c reduction for those with A1c > 8. There were also reductions in ER visits. There was a 30% decrease in diabetes-related visits and all-cause visits decreased by 55%. A reduction in costs of 55% from baseline to follow-up. Also, strong engagement of ~30 digital engagements per week suggests the tool is an effective mechanism to improve patient self-management of diabetes and increase in self-care behaviors.

To our knowledge, there are no EHRs which have reward/incentive capability.

We do not have many competitors doing the same as us. Our competitors offer traditional, one-time mailed gift cards (typically for a single quality measure such as Annual Wellness Visit) versus a full Financial Incentive Management tech-enabled service supporting complex care management and preventive care participation. Of course, we can also support the quality measure programs.

Differentiators vs EHR Functionality:

Welldoc believes that partnership is the pathway to better management of chronic conditions. Digital health solutions do not work in a silo and data and insights should be integrated into care models to support optimized interventions and clinical decisions. The Welldoc platform includes a digital ecosystem that allows for user coaching and support along with remote monitoring and tracking of patients. In addition, the platform includes integrated patient population management tools, dashboards and analytics.

Digital Coaching

Welldoc offers 1:1 asynchronous chat capabilities, two-way messaging, and live video meeting capabilities directly through our application. We enable this functionality for use by our clients’ Care Management teams, allowing them to function most efficiently in data-driven interventions that prioritize the members most in need.

Welldoc is a device-agnostic solution. The Welldoc platform connects to over 400 devices and data sources, including CGM, blood glucose meters, scales, blood pressure devices, fitness trackers, pharmacy and clinical laboratory data. Given Welldoc's device-agnostic approach, preferred supplies/consumable products are dictated and provided by the client. Welldoc can configure the app to support device/supply ordering functionality from within the application if so desired. This also prevents any interference with existing formulary strategies.

Integration into care models

Our Welldoc platform can push notifications, alerts, and reports to care team members and providers within their existing workflows with integration into their EMR or care management platform. Welldoc is capable of sending and receiving data to health systems. Integration details are handled at implementation. Welldoc has existing partnerships with Xealth and Redox to support technical integrations in line with your requirements. We will partner with you to develop and execute an integration approach that can seamlessly fit into your care model and support remote patient monitoring and tracking.

Client Success and Engagement Program

Welldoc also offers full client success and engagement support as part of our offerings to support outreach, awareness building, user activation, and overall engagement with our solution. We can work directly with you to build and execute a comprehensive, multi-channel campaign and communications approach in line with your program objectives and criteria. There are multiple benefits to Welldoc 510 K clearance and regulatory capabilities:

  • Efficacy: A class II medical device is often cleared along with clinical data and comes with indications for use, which helps target the use of the SaMD product in the right intended audience and discourages off-label use.
  • Device and Ecosystem Interoperability: FDA-clearance is a necessary factor in determining whether products can collect data directly from connected diabetes devices, such as BGM, CGM, and pumps. Real-time access to CGM can only be done between an iCGM (class II) and equivalent class II medical device, like Welldoc’s diabetes product BlueStar. BlueStar can collect data directly.
