  • Turn-Key: Inbound Health brings all capabilities and assets to launch and scale Rehab Care at Home.
  • Technology+: Inbound Health provides our technology platform, as well as a variety of consulting services, including but not limited to managed care and contracting support, implementation support, and supply chain coordination.
  • Tech-Only: Inbound Health provides our technology platform to enable Rehab Care at Home.

While structured in three levels, Inbound Health is open to discussions if none of the options fit the needs of health system partners. Similarly, Inbound Health understands that as these programs scale, hospitals and health systems may expect ownership of certain fuctions or capabilities to change, which Inbound Health is open to discussing and adjusting in a partnership year over year.

Inbound Health offers a comprehensive set of capabilities to hospitals and health systems designed to enable acute and post-acute care at home programs. Inbound's capabilities include home-based care pathway development, virtual care management, 24/7 biometric monitoring, in-home nursing and therapy visits, a proprietary technology platform (that supplements EHRs), supply chain coordination, clinical and operational oversight, and managed care support. 

The company's flexible partnership structure allows health system partners to leverage their existing assets and capabilities, while relying on Inbound to fill any and all remaining gaps to launch and scale advanced care at home programs. 

To date, Inbound Health has managed over 9,000 patients in the home across 350+ primary diagnoses.
