Description:
  • Atrium Health Wake Forest Baptist’s bariatrics department first deployed Carium to support patients on their medical weight loss journey. Their comprehensive program integrates remote monitoring, patient self-reporting, and care pathways into a data-assisted and personalized treatment program.
  • St. Luke’s, an Idaho-based, clinically integrated network of clinicians started using Carium to enable their health coaching program and within the first six months expanded to care management, employee wellness, lifestyle medicine, and cardiac rehab. St. Luke’s is leveraging Carium’s novel data to enrich their EDW and enable robust insights by combining biometrics, PROMs, clinical data, and claims data.
  • Atrium Health’s Perfect Care transforms cardiac care by engaging each patient and their family – in the comfort of their home – with a personalized approach. Carium’s innovative tech connects patients and their care teams to monitor, engage, and proactively collaborate with patients before their cardiac surgery and during their recovery journey.
  • A primary care practice based in California, Lifestyle Medical, quickly pivoted during the pandemic to address the care needs of their patients — from the safety of home — with remote patient monitoring (RPM) and virtual patient engagement. With strong patient experience results, their practice recently expanded Carium as their digital front door.
  • Health Logx partners with primary care practices to provide fully virtual health and wellness services for patients, on behalf of the practice. They focus on RPM and chronic care management (CCM) programs to track patients in between office visits. Patients are equipped with connected devices to gather biometric data and interact with Health Logx via messaging, video chats, and phone calls.
  • The Mecca Gym isn’t a typical gym or weight room. It’s all about fostering personalized experiences, building accountability, and scaling resources to promote health among members. Their personal trainers—with advanced degrees and clinical credentials—regularly track member behavior and progress using Carium.

Today, we act as the eyes and ears on the ground for nephrologists and PCPs -- extending their reach and supporting patients in between appointments to drive the best outcomes. We provide patients with a highly specialized multidisciplinary care team (includes a peer mentor, someone who has lived with kidney disease, a renal pharmacist, a dedicated dietitian and a social worker), which is a layer that most health systems and ACOs cannot provide today given the nuances of this complex disease

EHRs, due to their ubiquitous presence in healthcare, are also considered competitors. However, their primary function as digital record-keeping fails to deliver action-based engagement on the data it stores.
