Wellpepper is an enterprise platform for interactive care plans. We support any type of patient journey, and deliver patient experiences through different modalities including mobile, web, SMS, email, and voice. Since patients are often being treated for more than one issue, and since health systems need to manage costs and complexity, Wellpepper enables care plans built from reusable building blocks so that a single system can deliver care for pre and post-acute, and chronic conditions. Care plans can be simple survey interactions or complex multi-activity experiences, and can be based on industry-leading clinical best practices like those from our partner Mayo Clinic, or can be designed to deliver best practice care plans based on a health system’s own patient workflow. We support over 40 care play scenarios including pre and post-acute and chronic disease management, so patients may use the system for a short time to manage an event or indefinitely. On the clinical side, the most active users are those responsible with supporting the patient including nurses, physician assistants, medical assistants, care managers, health coaches, physical therapists, dieticians, and others.
