Our KodaCares service layers on top of our digital tools to provide a human touch for your patients who need additional support. Our expert team guides them through their planning but also works to alleviate any burdens from plan completion - from facilitating conversations with their care team and family members, to clarifying therapeutic options.

Koda's ACP allows patients to digitally send their plans to their Surrogate Decision Makers (SDMs) and any other caregivers. Our KodaCares service facilitates phone calls between SDMs, loved ones, and the patient to ensure clarity and alignment regarding care preferences.

Koda seamlessly fits within a system's workflow and needs. Once patients are referred to us, Koda's ACP experience guides them through their planning. Once their plans are complete, they're made digitally available to your organization. You'll receive regular updates about the progress of your patient base through ACP; we also notify you of patients appropriate for a DNR-OOH, and any needs that may become apparent while your high-risk patients are supported through our KodaCares service.
