Data is available to the healthcare team through a provider-facing dashboard, and high risk situations are actively communicated to the patient’s healthcare team, enabling recognition of hypertension risk when it occurs and informing any necessary changes to the care plan, including the option to enroll patients onto Babyscripts myJourney Blood Pressure RPM.

Patients track their blood pressure readings at home with the support of a dedicated care manager.
