Fabric's Intake & Care Guides improves the experiences of both patients and providers by eliminating duplicate tasks, reducing nurse documentation time, and actively engaging and informing patients throughout their visits. Subflow has the ability to use multiple delivery options in order to maximize the choices for not only the provider, but in particular, the patient - allowing them to receive and digest important care journey information - and that action, or inaction, is captured by Subflow.

Subflow delivers content relevant to a patient/member/clinical trial participant determined and created by the clinician - targeted just for them and delivered when they need it.

Subflow's ability to help a patient navigate their own care is made even more efficacious by our ability to capture their adherence behaviors and interactions with the recommended approach, once they leave a provider's building.

Subflow delivers information determined by the provider designed to better personalize, inform, and guide the next steps in a patient's care journey and whether there is a need for targeted intervention, based upon current adherence actions. Because we enable personalized materials to patients to learn about the risks and benefits of different options, form preferences, and share in the decision-making on treatment options, the overall cost of care can be lowered, and outcomes improved.

Whether in written or via video (not third party hosting), Subflow will deliver automatically based upon a customized workflow - all educational materials at a time determined by the clinician.

Subflow reduces steps for physicians prescribing education to patients and aligns with existing workflows (e.g. content suggestions per the patient's health history, favorites tagging, and folder access). Care managers can easily access content, while marking their favorites and organizing it into folders shared across the team and so they can easily see what other managers have shared with patients too. Care coordinators can find and use relevant health information in real-time, in preparation for a call, during an encounter, or to send as follow-up.

Subflow listens for trigger events, whether using an EHR or even CRM, to initiate automated workflows to facilitate care navigation, capturing patient behaviors (medication adherence, education material review, etc) so a patient's cost of care is reduced due to their increased participation.
