Description

Product Description:

TriageGO is a software tool that leverages machine learning to suggest an acuity-level at triage, based on the likelihood of various clinical outcomes. The tool supports the nurse by providing an actionable data point and, combined with their own clinical judgement, enables the nurse to sort patients into the correct trajectories for care in the Emergency Department. TriageGO empowers the clinician to better-differentiate patients based on their clinical risk, and in doing so, has impactful operational benefits; decreasing time-to-provider, time-to-emergent care for high-risk patients, and time-to-discharge across the board. TriageGO is seamlessly embedded within the EHR (Epic & Cerner) so that it is not disruptive to the clinician's workflow.

Product Capabilities

Bright.md is accessed via SSO with EHR portal credentials. Patient interviews are available on-demand from a mobile or desktop with just a 3G connection, in English and Spanish.

Bright.md provides an asynchronous virtual care solution in which a patient completes an online symptom assessment, which is reviewed by a provider who recommends a treatment plan and diagnosis. Data required for treatment is collected via a series of clinical interview questions that the patient provides answers to via multiple choice, open text, or by uploading a photo relative to their concern. Allergies, medications, and problem list for the patient is brought in from the EHR and made visible to the provider who is providing treatment.

Bright.md translates the patient interview into a chart-ready SOAP note and makes it readily available via MDM interface for integration as an encounter in the EHR. Bright.md presents all necessary information for the provider to confirm the diagnosis, change the diagnosis, or escalate the patient to a higher modality of care if they feel that treatment cannot be provided asynchronously.

Technical support is provided to our health system customers directly. We offer 24/7 support for our provider users and program administrator users.

Bright.md is staffed by health system providers, typically using a centralized care team that manages virtual visits. We can support multiple provider pools that may specialize in certain types of conditions, so patients awaiting treatment are segmented by condition.

Bright.md provides in-depth reporting to our customers to measure their utilization and success over time. Specific reporting metrics include: Provider efficiency Interview utilization Common diagnoses Patient satisfaction and feedback Prescription volumes Escalation rates

Bright.md provides the software solution for health systems to treat 130+ diagnoses virtually, without the use of video (asynchronous telehealth). Patients complete an on-demand interview at any time that captures a thorough symptom assessment while ruling out potentially emergent conditions, just like a face-to-face visit with a clinician. Patients receive questions based on their answers, and if an emergent condition is suspected, the patient is escalated out, with recommended next steps configured by your system. Clinical decision support provides a recommended diagnosis and treatment plan derived from evidence-based guidelines, along with a SOAP formatted chart-note logged as an encounter in your EHR—allowing providers efficiency while preserving autonomy and clinical judgment. Once the provider completes the asynchronous visits, the patient receives a complete after visit summary with treatment instructions and education, along with any prescriptions sent electronically to their preferred pharmacy.

Use Cases

Description:

When a patient comes into an Emergency Department (ED), they are triaged into one of five acuity-levels; 1 being high-acuity, likely in need of intensive care or emergency intervention, 5 being low-acuity, not likely to need intensive care, emergency intervention, or hospitalization, and therefore likely to be discharged home. The current methodology of triage in most United States EDs is a manual desicion tree based on anticipated resource-utilization. However, using this system, the majority of patients are assigned to an acuity-level 3, with poor differentiation based on their actual clinical risk (as resource utilization is a poor proxy for clinical risk). This can make it very difficult for clinicians to properly identify which of these mid-acuity patients needs care most urgently, and often creates a clutter effect causing increased wait-times in the ED.

When TriageGO is installed within the triage workflow in an ED, it helps the nurse to discern a patient's actual clinical risk, and prioritize them appropriately. When patients are sorted into the correct trajectories for care immediately upon being triaged, the clutter effect is reduced. EDs who use TriageGO observe decreased time-to-provider, time-to-ICU, time-to-OR, time-to-admission, and time-to-discharge.

Pediatric use cases:

Pediatric application of TriageGO is the same, except the model is adjusted for pediatric clinical presentations.

Users:

Emergency Nurses, Emergency Physicians, Emergency Dept Administrators

Differentiators

Differentiators vs EHR Functionality:

TriageGO embodies the principles of Open Innovation fostered by EHRs like Epic & Cerner who support an app-market-style ecosystem. While some risk-predictive features exist within current EHRs, there is no comparable solution for risk-stratification at triage, trained on the data within that hospital's instance of the EHR.

Differentiators vs Competitors:

There is one similar product on the market known as KATE from Mednition. This product still relies in the resource-based ESI methodology of triage. One hospital using KATE reported just 65% accuracy in triage asignment. In an evaluation done at Johns Hopkins, where TriageGO was invented, the tool demonstrated 78% sensitivity to identify patients who went on to need critical care, and a 120% improvement over ESI to identify those who went on to need an emergency procedure. It was also observed that using TriageGO, the rate of low-acuity patients who were ultimately hospitalized went from 6% to 3%.

Differentiators vs EHR Functionality:

Our content is evidence-based and updated regularly based on the latest clinical guidelines. It is developed, constantly evaluated and updated based on user data and feedback, and maintained by a team of physicians, writers, and editors. Our Interview is available in English or Spanish with medically-certified translation, and all of our content is created at a 4th-grade reading level, removing medical jargon and using images as easy guides.

Because we've been building and refining our clinical content for more than eight years, improving with reviews and feedback from clinician leaders at some of the top health systems in the country, we've built a clinical content engine that cannot be replicated.

One of the Lead Clinicians at one client shared that he reviewed all of our content in detail and was "extremely impressed", sharing that it would "take thousands of hours of clinician time to even attempt".

Differentiators vs Competitors:

Robust, Proprietary Clinical Content

With Bright.md, your providers are reviewers of content, not creators. Our evidence-based content containing 29 unique interviews, and treating 130+ diagnoses, is developed and maintained by a team of board-certified physicians to keep yours focused on delivering care to patients.

Self-Service Administration

Respond quickly to changing demands with real-time access to custom care routing, interview activation, and more.

Bi-directional EHR Integration

Drive long-term care delivery efficiencies with asynchronous visits that are integrated into your clinical operations workflow.

Bright.md uses standard HL7 protocols and FHIR APIs to sync information that decreases the administrative burden on providers, while creating easy access for patients.
